Job summary

The post holder will be an experienced Advanced Nurse Practitioner (ANP) or Urgent Care Practitioner (UCP) who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. You will demonstrate safe, clinical decision-making and expert care for patients registered with the practice. You will be employed by the PCN to work across any of our five member practices, however, your primary base will be at Bramhall Park and Shaw Heath Medical Practices.

Main duties of the job

We are seeking an experienced clinician to assess/triage a range of same-day and chronic patients, offering advice, care planning, treatment and prescribing in line with NICE, local guidelines, PGDs/PSDs and within scope of practice. Youll manage undifferentiated conditions, identify red flags, refer appropriately, and use advanced clinical judgement to support diagnosis and treatment planning.

The role requires a personalised, population-centred approach, supporting shared decision-making and holistic care. You will undertake face-to-face, home and remote consultations (phone/video), including visits to patients in residential/sheltered settings. Youll provide safe, evidence-based prescribing, initiate diagnostic tests, interpret results, and deliver cost-effective, individualised care.

Youll supervise and support a range of primary care roles (e.g. FCPs, junior nurses), complete all mandatory/local training, and support care home initiatives. The role also involves contributing to quality targets, service development, health promotion, and multi-professional education. Autonomous, accountable practice and strong teamwork across the PCN is essential.

Assess / triage patients, including a range of same-day access / chronic patients. Provide advice, treatment, care planning and review I line with patient health needs. This will include prescribing a range of medications within your scope / policy and under PDGs , PSDs, NICE, local clinical guidelines or pathways. Make referrals to other members of the primary care team or secondary care services where appropriate.

Manage a range of patients with previously undifferentiated diagnosis identifying any red flags or underlying serious pathology and take appropriate action.

Use complex decision-making / clinical skills and experience to inform the potential diagnosis, investigation, and complete management of episodes of care within a broad scope of practice.

Actively take a personalised care approach and population-centred care approach to enable shared decision-making with the presenting person.

Complete all mandatory training in line with both local and national policy.

Complete the relevant clinical / local training in order to provide multi-professional clinical practice and CPD supervision to other roles within the primary care team, such as first contact practitioners, medicines management staff, junior nursing staff, care co-ordinators and well-being practitioners.

Undertake assessment on site, in the patients home (or other care setting) or via any digital platform or telephone system utilised by the Practice / PCN.

Make home visits to those who are too unwell to come into the clinic in person including those patients living in sheltered / supported / residential accommodation where appropriate.

Support the PCN / Practice care homes in partnership with the wider practice team to ensure safe effective care of patients in residential care.

Make professionally autonomous decisions for which you are accountable in line with local / national policy.

Provide assistance to GPs / other clinical staff when seeing and treating patients as appropriate.

Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.

Prescribe safe, effective and appropriate medication as defined by the current legislative framework in accordance with the Practice prescribing formulary whenever this is clinically appropriate. Avoid the use of grey drugs.

Provide safe, evidence-based, cost-effective, individualised patient care.

Offer a holistic service to patients and their families, developing where appropriate an ongoing plan of care/support, with an emphasis on prevention and self-care.

Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.

Refer patients directly to other services/agencies as appropriate.

Work with the team within the Practice, and across the wider NHS, to promote integrated and seamless pathways of care.

Contribute to the practice achieving its quality and performance targets to sustain the high standards of patient care and service delivery.

Participate in the identification of community health needs and develop patient/family-centred strategies to address them.

Help develop and set up new patient services and participate in initiatives to improve existing patient services.

Offer support, supervision and training to junior members of the team.
