Main duties of the job

The Patient Services Care Co Ordinator will work as part of the PCN Care Co Ordinator team and the Practice team and will be based at Bramhall & Shaw Heath Medical Group, one of the PCN GP Practices.

Job responsibilities

Purpose of the role:

  • Improving patient accessandreleasing GPsto concentrate on treating and managing patients with more complex conditions.
  • Improving patient flowwithin surgery hours, increasing the time efficiency of appointments, and reduction of waiting times; supporting patient experience by translating or reiterating information from the GP
  • Reducing complaints and efficiently responding to any received
  • Set up patient participation and support groups.
  • Reduce time spent by GPsand the Management team on managing correspondence.
  • A variety of Practice administration

The Patient Services Care Co Ordinator will work as part of the PCN Care Co Ordinator team and the Practice team and will be based at Bramhall & Shaw Heath Medical Group, one of the PCN GP Practices.

Main duties and responsibilities

Be responsible for managing the clinical appointment books and cover including booking locums; collating all necessary compliance documentation from locums.

Monitor, maintain and update the clinical system appointments making sure there are correct appointments on the system that match the rota and slot types for online booking for patients.

Organising the efficient deployment and utilisation of staff working within the Practice across administrative tasks to ensure its effective functioning and to maximise efficiency.

Liaise with patients to form a Patient Participation Group, arrange meetings, surveys and engage with the group to get ideas how to improve services at the practice.

Support with Quality Improvement, Quality Outcomes Framework, Impact and Investment and LCS (Locally Commissioned Service Scheme) work for the Practices.

Supporting uptake work and screening recalls linking to the above frameworks

Be responsible for managing the call and recall process outside of the above frameworks.

Sorting all clinical post, reading, coding and prioritising for the GP in terms of actions. Signposting some post to others such as clinical pharmacist etc.

Extracting all information from clinical letters that needs coding and adding to notes.

Arranging appointments, referrals, tests and follow up appointments of patients.

Following clinical pathways as per practice protocols

Assist with CQC compliance for the Practice.

Support the Practice Leadership and administrative teams on a variety of appropriate support tasks as and when required.

To work collaboratively with Patient Service care Co-ordinator A to aid cross cover.
