Job summary

We have a vacancy within our Care Navigating Team and are looking for the right person to join this busy and rewarding environment.

Our Care Navigators are the first point of contact for patients contacting the surgery. They play a key role in ensuring patients are directed to the most appropriate service to meet their needs. This role is both demanding and fast paced, but equally rewarding.

No two days are the same, and you can expect your days to pass quickly in this varied and important position. Our Care Navigators are vital to the effective running of the surgery and to ensuring patients receive the right care at the right time.

We are looking to recruit someone who has experience working within general practice and who is confident communicating with patients while managing a high volume of enquiries in a busy environment.

Main duties of the job

Key Responsibilities

  • Work as part of a busy team to support patients in accessing the most appropriate healthcare professional for their needs, including directing them to both internal and external services where required.

  • Handle a high volume of telephone calls, ensuring an excellent telephone manner and delivering high standards of customer service at all times.

  • Manage the reception area by welcoming patients and visitors in a professional, friendly, and courteous manner.

  • Liaise with external agencies, take accurate messages, and provide administrative support to colleagues when needed.

  • Use IT systems to book appointments, make patient referrals, and access information via the practice intranet.

Farnham Medical Centre is a large, well established GP practice located in South Shields, caring for over 17,000 patients. We are a busy and friendly practice with a strong commitment to providing high quality patient care.

Our clinical team includes Doctors, Advanced Nurse Practitioners, Minor Illness Nurses, Practice Nurses, Health Care Assistants, Phlebotomists, and First Contact Practitioners. They are supported by a dedicated team of Managers, Care Navigators, Administrative Staff, GP Assistants, and Secretaries who work together to ensure patients receive the best possible service.
