Job
Summary
To take a lead in nursing services to patients
of the practice including treatment, screening, advice and management of
patient groups with chronic diseases within the professional competence of the
post holder.
The nurse will work to mutually defined
protocols and establish programmes of care for patient groups with continuing
overall nursing responsibility, supervising, supporting and teaching other
practice nurses and members of the practice team.
The nurse will work in accordance with the NMC
Code of Conduct.
Roles and
Responsibilities
Develop specific clinics or services to meet
the identified needs of patients with a focus on quality, range and accessibility
of services, including care in the home and nursing and residential homes.
To develop, initiate and manage nurse led
ambulatory care clinics, including dressings, suture removal, ear syringing
etc.
To lead on establishing Chronic Disease
Management within the practice, including calls and recall systems, diagnostic
tests, (eg venepuncture, ECGs, spirometry etc.) audit and liaison with other specialist
practitioners to create a unified strategy for people with long term
conditions.
To be a primary point of contact and have an
active role within the practice for womens health including cytology, family
planning and menopause management.
To be a primary point of contact and have an
active role within the practice for vaccination and immunisation programmes,
including adult, adolescent and children and babies.
To have an active role in health promotion and
public health with the primary health care team.
Work with GPs and other professionals to
continually develop clinical skills, standards, protocols, guidelines, patient
group directions to ensure safe and appropriate care.
Ensure the delivery of nursing care within the
practice is evidence-based and that practice is clinically reviewed through
episodes of reflection and clinical supervision.
Ensure the needs of the practice population are
being addressed through co-ordinated input with the multi-disciplinary team.
Participate in the development and
implementation of new initiatives within primary care.
Manage, support, supervise and teach other
practice nurses and health care support workers within the practice.
Ensure the developmental needs of practice
nurses and health care support workers are formally identified and endeavour to
create opportunities to meet these needs.
Demonstrate commitment to life long learning
linked to the changing needs of patients
Actively engage in multi-disciplinary and
multi-agency referral processes
Lead in audit processes and incorporate patient
involvement in standard setting
Attend and participate in staff meetings, case
conference and other meetings as required.
Ensure personal professional status by ensuring
current NMC requirements are met including current registration and indemnity
with a professional organisation.
Assist in the maintenance of a safe environment
and identification of potential risks for all patients and personnel within the
practice setting
Comply with Health and Safety regulations fire
regulations and complaints procedures in accordance with PCT guidelines
Demonstrate active participation in clinical
governance.
Ensure clinical equipment and supplies are
maintained and stored according to guidelines.
