The Valleys Primary Care Network (PCN) is looking to recruit an experienced, proactive Clinical Pharmacist to provide clinical leadership on medicines optimisation and quality improvement. Passionate about high quality patient care, you will work as part of a multi disciplinary team in a patient facing role. The position is offered on a part time basis approx 28.5 hours per week. Salary negotiable depending on experience.
We currently have a team of 7 Clinical Pharmacists and 5 Pharmacy Technicians serving The Valleys patient population (circa 58,000). Based at one of the 6 member practices within The Valleys PCN, you will work within your competency to provide medicine optimisation expertise to our practices and to our patient population. This is an evolving clinical role with scope for professional development and the opportunity to develop a specialised area of interest.

Main duties of the job
- Clinical medication reviews for patients with single or multiple long-term conditions where medicines optimisation is required
- To be involved in multidisciplinary clinical meetings within the PCN where appropriate
- To manage repeat prescription requests, resolving queries where possible within scope of practice. To ensure patients have appropriate monitoring tests in place when required.
- Implement changes to medicines in line with MHRA alerts, product withdrawal or shortage and other local or national guidance.
- To maintain full and complete records of all patient contacts using appropriate clinical templates and coding
- To undertake a proactive role in audit and quality improvement implementing recommendations where appropriate.
- Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate.
- To monitor network practices prescribing against the local health economy and make recommendations for prescribers or amend within scope of practice, based on findings.
- Work with practices to ensure full compliance with Care Quality Commission standards for safe and effective care.

Working to deliver services to a population of circa 57,000 patients, the PCN strives to be innovative and deliver on both local and national priorities. The PCN consists of 6 member practices (Meltham Group Practice, Slaithwaite Health Centre, Oaklands Health Centre, Elmwood Family Doctors, Colne Valley Group Practice and Honley Surgery) two of which are rated Outstanding by CQC. Some practices within the PCN are involved in clinical research and many are training practices. The PCN covers the semi-rural populations across the Holme Valley and Colne Valley area of Huddersfield.

Providing clinical leadership on medicines optimisation and quality improvement, the post holder will also manage some aspects of the quality and outcomes framework and enhanced services. The development of systems to effectively manage medicines upon transfer of care into primary care or care homes is also a requirement of the post. The post holder will work to develop systems for safer prescribing and share innovations across the PCN practices, as well as the provision of clinical pharmacist functions at a practice level.
The post holder will be an experienced Clinical Pharmacist, who acts within their professional boundaries to support the development and mentoring of the pharmacy team.
The post holder will hold an independent prescribing qualification or is working towards the qualification.
The post holder will be encouraged to specialise in an area of focus aligned to delivering the PCN priorities.
