Job summary
North Care Network is seeking an enthusiastic, motivated and
committed Advanced Clinical Practitioner (Nurse or Paramedic) to join our
vibrant and welcoming PCN. Post Graduate qualification in Autonomous Practice
is essential and a non-medical prescribing qualification is also essential. As

Main duties of the job
To perform as an autonomous practitioner working independently across
our PCN and with other health care professionals. Managing daily surgeries of
face to face, online and telephone consultations to assess, diagnose and treat
patients with a range of acute, non-acute and chronic medical conditions,
initiating direct referrals as appropriate.

Assess undifferentiated
patients with a range of acute, non-acute and chronic medical conditions.

differential diagnoses and carry through management treatment plans
ensuring relevant follow up.
Discuss with registered
GP and, where appropriate, make referrals to other health professionals in
primary and secondary care.
Arrange hospital
admission for appropriate cases.
Undertake telephone
triage work, telephone advice calls and online consultations. Be part of extended access provision and to provide care to those patients in the network who have difficulty in attending during core hours.
Managing and undertaking
home visits including Care Home visits Managing Palliative Care
patients, prescribe medication as
necessary as a non-medical prescriber.
Maintain accurate record
keeping in SystmOne/EMIS using templates and READ codes.
Undertake regular
medication reviews and medicines management Request and interpret
tests appropriate to patients needs.
Manage
communications from hospitals and other providers e.g. Immediate Discharge
Summaries acting on requests for further action, medication etc
