Job summary
We are looking for a dedicated Clinical Pharmacist to become a key
member of our multidisciplinary team, collaborating with GPs, nurses,
pharmacists, and pharmacy technicians to deliver high-quality pharmaceutical
care. This role is ideal for someone who is compassionate, proactive, and
self-aware, with excellent communication and problem-solving skills. You will
work directly with patients to optimise medication management, support chronic
disease care, and contribute to safer prescribing practices.
Main duties of the job
Clinical pharmacists are an integral part of the general practice team, working to improve outcomes from medicines while ensuring patient safety and value. In this role, you will consult with and treat patients directly, providing support for those managing long-term conditions, offering advice to patients on multiple medicines, and improving access to health checks.
Key responsibilities include providing expertise in practical therapeutics, developing bespoke integrated care plans, and facilitating communication across the patients care pathwaylinking with primary care, secondary care, social care, and domiciliary services. You will manage patients with complex long-term conditions such as resistant hypertension or lipid therapy adherence, and help increase the safe and effective uptake of new medicines.
This role takes a holistic approach to medicines, supporting step-up and step-down management for long-term conditions and end-of-life care.

In this role, you will deliver high-quality patient care by conducting clinical assessments, providing both telephone and face-to-face structured medication reviews, and managing patients with complex polypharmacy, particularly elderly individuals and those with multiple comorbidities. You will apply your pharmaceutical expertise to develop personalised care plans, support medication adherence, and promote safe prescribing practices. Collaboration is key, and you will work closely with GPs, nurses, and other healthcare professionals to strengthen patient care pathways and ensure seamless transitions of care. The majority of work for this role is centred around care homes.
You will also play a central role in chronic disease management, overseeing long-term conditions and optimising treatment for patients with hypertension, lipid disorders, and other health needs. Alongside patient-facing work, you will contribute to education and support by delivering patient education, training colleagues, and engaging with clinical research and innovation. Finally, you will help drive operational efficiency by managing prescription processes, responding to medication-related inquiries, and supporting quality improvement initiatives such as QIPP, QOF, and other financial initiatives offered by commissioning bodies.
