Primary Care Network Clinical Pharmacist (Band 7)

Job summary

We are looking for a dedicated Clinical Pharmacist to become a key member of our Primary Care Network multidisciplinary team. The successful candidate will work as part of a pharmaceutical team (another clinical pharmacist and pharmacy technician) delivering Network Directed Enhanced Service to two member practices. Furthermore, additional projects will include collaboration with practice GPs, Nurses, and other practice staff ensuring high quality support of our patients. This role is ideal for someone who is compassionate, proactive, relatively junior in this field of expertise, but must possess excellent communication and problem-solving skills. You will work directly with patients to deliver structured medication reviews, support chronic disease care, and contribute to safer prescribing practices.

Main duties of the job

Patient Care: Conduct clinical assessments, provide face-to-face and telephone structured medication reviews, and manage patients with complex polypharmacy, especially the elderly and those with multiple comorbidities.

Pharmaceutical Expertise: Develop personalised care plans, support medication adherence, and facilitate safe prescribing practices.

Team Collaboration: Work closely with GPs, nurses, and other healthcare professionals to enhance patient care pathways and ensure seamless transitions of care.

Chronic Disease Management: Oversee long-term condition management, including medication reviews and optimisation for patients with conditions such as hypertension and lipid disorders.

Education and Support: Participate in patient education, provide training to colleagues, and support innovation in clinical research. The PCPEP Course will be a mandatory commitment in line with NHS England direction.

Operational Efficiency: Respond to medication-related inquiries, and support quality improvement initiatives such as QIPP, QOF, and other financial incentivised projects.

  • A minimum of two years experience as a pharmacist.

Qualifications

  • Undergraduate degree in Pharmacy with registration by the General Pharmaceutical Council (GPhC); Experience managing acute and long-term conditions commonly encountered in general practice; strong problem-solving abilities with an understanding of when to escalate to senior professionals.

Desirable

  • Essential Qualifications and Skills: Postgraduate qualifications, such as a Masters in Pharmacy (MPharm) and/or completion of the Primary Care Pharmacy Education Pathway Course.
