Job summary
We are looking for a dedicated Clinical Pharmacist to become a key
member of our multidisciplinary team, collaborating with GPs, nurses,
pharmacists, and pharmacy technicians to deliver high-quality pharmaceutical
care. This role is ideal for someone who is compassionate, proactive, and
self-aware, with excellent communication and problem-solving skills. You will
work directly with patients to optimise medication management, support chronic
disease care, and contribute to safer prescribing practices.
Main duties of the job
Patient
Care: Conduct clinical assessments, provide face-to-face structured medication
reviews, and manage patients with complex polypharmacy, especially the elderly
and those with multiple comorbidities.
Pharmaceutical
Expertise: Develop personalised care plans, support medication adherence, and
facilitate safe prescribing practices.
Team
Collaboration: Work closely with GPs, nurses, and other healthcare
professionals to enhance patient care pathways and ensure seamless transitions
of care.
Chronic
Disease Management: Oversee long-term condition management, including
medication reviews and optimisation for patients with conditions such as
hypertension and lipid disorders.
Education
and Support: Participate in patient education, provide training to colleagues,
and support innovation in clinical research.
Operational
Efficiency: Manage prescription processes, respond to medication-related
inquiries, and support quality improvement initiatives such as QIPP and QOF.
