Job summary

Join our team as a Health and Wellbeing Coach, where you can empower individuals to proactively manage their physical and mental health. You will help clients build knowledge, skills, and confidence to improve their health outcomes and quality of life. Through multiple coaching sessions, you will assist them in setting and achieving personalised health goals. You will manage and prioritise a caseload based on individual needs, providing interventions like self-management education and peer support. Additionally, you will collaborate with other health professionals when necessary. Be a part of transforming lives and making a real difference!

Main duties of the job

The successful candidate will be kind, reflective and self-aware and will enjoy working with a wide range of people. They will have good communication and negotiation skills and a firm belief that people have untapped resources within them, that can be unleased by providing a high quality, non-judgemental service. They will be proactive with a flexible attitude, keen to work as part of a team and committed to developing themselves in this skilled role and supporting colleagues to do the same.

This role focuses on enhancing 'patient activation' by empowering individuals to improve their health outcomes and sense of wellbeing, thereby reducing unnecessary dependence on clinical services. This is achieved through interventions like self-management education and peer support, helping individuals set and achieve personal health goals. Collaborating with social prescribing services ensures appropriate referrals to community-based activities that support health and wellbeing. As part of a multidisciplinary team, the role promotes Health Coaching and advocates for Personalised Care and Supported Self-Management, guiding local professionals in making suitable referrals and raising awareness of health coaching services, particularly among underserved groups. Team participation and pathway development tailored to patient needs are essential, as is holistic support in collaboration with health, social care, and voluntary sector providers. Ensuring all PCN staff are knowledgeable about health coaching and social prescribing, fostering shared decision-making, and understanding patients' activation levels are key responsibilities. The role involves facilitating access to personal health budgets when suitable, using IT and MDT channels to identify patients who would benefit from health coaching, and adapting to service needs. Additionally, it includes gathering and analysing data on service user experiences and the impact of health coaching to support the integration of personalized care into local health systems.
