Job summary

Hungerford Medical Centre is seeking a proactive and compassionate Care Coordinator to join our team. The role will support the delivery of person-centred care by working alongside our existing Care Coordinators to identify and address the non-clinical need of patients with complex health and social circumstances. The post holder will provide operational and administrative support to the Care Coordination function. You will also provide cover during periods of absence within the Care Coordination team, ensuring consistency of service delivery across practices.

  • Care co-ordinators help individuals make the right connections with the right teams at the right time,particularly for those with long-term conditions or frailty.

  • They focus on what matters to individuals and support people from diverse backgrounds, including those with various conditions and disabilities.

  • They assist with administrative tasks such as booking appointments, managing clinical records and generating reports.

  • Care Coordinators work closely with GPs and other primary care professionals

Main duties of the job

  • Managing the learning disability register
  • Managing the weekly smears register
  • Setting up and running the new starter/leaver process
  • Managing the gender clinic spreadsheet
  • Overview of the registration workflow ensuring all work is completed
  • Involved in producing insurance reports and SARS requests
  • Cover for Prescription Clerks with prescription administration as and when required.
  • This is not a limited list of duties and other ad hoc areas may be introduced

Service Delivery

Staff at Hungerford Medical Centre must adhere to the information contained with practice policies and regional directives, ensuring protocols are adhered to at all times. Staff will be given detailed information during the induction process regarding policy and procedure.

Security

The security of the practice is the responsibility of all personnel. Staff must ensure they remain vigilant at all times and report any suspicious activity immediately to their line manager. Under no circumstances are staff to share the codes for the door locks to anyone and are to ensure that restricted areas remain effectively secured.

Professional Conduct

At Hungerford Medical Centre, staff are required to dress appropriately for their role. Administrative staff will be provided with a uniform whilst clinical staff must dress in accordance with their role.

  • Ability to work with confidential information and maintain records accurately
  • Proficient in Microsoft Office and digital systems

Desirable

  • Good understanding of primary care and community health services
  • Basic understanding of care coordination or personalised care
  • Experience working with vulnerable patient groups
