Job summary

Experienced Practice Nurse required for to work at our Modern General Practice site in Ascot.Part-time for 3 days a week.Long-term locum agreements can also be considered.

You will be joining a dynamic and forward thinking team based at the Heatherwood Hospital site using Data driven ways of working, risk stratifying and targeting patients for the right clinician at the right time to help solve problems and make a difference to the patient and population as a whole.

Main duties of the job

Duties will include management of chronic illnesses including diabetes, cytology, vaccinations and overall general treatment services. Working as part of the practice multidisciplinary team, delivering care within their scope of practice and in line with national guidelines to the entitled patient population.

The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, disease prevention, chronic disease management, immunisations, cytology as well as supporting the management team in the reviewing of clinical policy and procedure.

JOB DESCRIPTION:

* Childhood Immunisations

*Long term Conditions Reviews - eg., COPD / Asthma / Diabetes

*Basic wound care

*Contraception Reviews

*Removal of sutures

*Peak flow readings

*Advanced: Spirometry. Undertake spirometry / reversibility as requested by a GP

*Assisting GPs with minor surgery / coil fittings

*ECG Recording. Carry out ECG's for interpretation by a GP

*Blood pressure measurements

*Routine immunisations

*Urinalysis

*Administration of medication. Flu vaccinations, Pneumococcal, Shingles vaccinations, B12, Depo injections

*Venepuncture (desired but not compulsory).Take blood samples as requested or in line with practice procedures.

*Non-clinical - Admin. Infection Control, stock ordering (medical)

*Home visits. Visits with supervision from Practice Nurse for housebound/Care Home patients to carry out the above duties as required.

*Audit. Participate in practice audit as directed by the audit lead

*Local initiatives. Participate in local initiatives to enhance service delivery and patient care

*Medical Students. Accommodate medical students observing clinics and provide demonstrations as requested by GP

*Chaperone. Act as a chaperone as required.

*Health Promotion. Provide lifestyle advice.

*Continuing Professional development. Support and participate in shared learning within the Practice. Performing other related duties as assigned by the Practice Manager or Clinical Supervisor.
