Are you a Paramedic Practitioner and non-medical prescriber (V300)?or willing to do the prescribing module? Do you have community/primary care/urgent care experience and have been working at an advanced level autonomously? Are you looking to develop your skills? in Primary Care, then this is the role for you!

We are looking for Paramedic Practitioners to work within our newly developed neighbourhood team.

Main duties of the job

You will be at the heart of our primary care network program to support vulnerable patients. Proactively working with practices, other voluntary agencies and our advanced nurse practitioners to identify and care for vulnerable patients in care homes and the local community. Such as those who have complex medical issues, palliative care patients, those who have Learning disabilities, Autism , Serious mental health illness and those who struggle to access medical services directly from a GP practice and would benefit from support around managing their ongoing care. You will visit patients in care homes, review and assess their health and well-being needs, design and agree with the patients a personalised care plan. Then work with patients, their families, carers and the wider Multi-disciplinary team within community and secondary care to ensure that they can live the best possibly life within the boundaries of their medical limitations.

About us

Rural rother PCN is based in an Office in Battle East Sussex. We have an excellent team of clinical pharmacists, physios, Advacned nurse practitioners, social prescribers and care navigators that all provide a service to our patients in the rural rother area.Our surgery teams are working closely with each other, enjoying the ability to share expertise and resources, to develop new services. Our vision is to continue to improve the quality of care that we provide in alignment with the need of our patient population.

Primary Responsibilities

The following are the core responsibilities of the Clinical operation lead. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

Supporting and caring for patients in the community

Assess, create or update personalised care plans for vulnerable patients living out local primary care network area of Rye, Camber, Winchelsea Sedlescombe, Westfield Northiam, Broad oak Robertsbridge and Burwash

Triage and manage acute and non- complex medical needs of patients who are housebound, palliative care and vulnerable, within your scope of practice and knowledge. Where other clinical support is needed refer to GP or seek appropriate support of other community basis services.

In conjunction with our Advanced Nurse Practitioners, work with other local service providers such as GP surgeries, community services, secondary care and voluntary sector to ensure that there good communication between the multi-disciplinary services.

Manage patients presenting with a range of chronic medical conditions, providing subject matter expert advice or support patients to access care for these issues

Implement and evaluate individual specialised treatment plans for chronic disease patients

Identify, manage and support patients at risk of developing long-term conditions with the aim of preventing and managing the adverse effects these disease may have on the patients health

Provide advanced, specialist nursing care to patients as required in accordance with evidence based evidence, NICE and the NSF

Support the undertaking of simple diagnostic procedures such:-

Undertaking of ECGs and blood pressure monitoring

Undertaking observations including monitoring for sepsis

Undertake the collection of pathological specimens

Request pathology services as necessary

Process and interpret pathology and other test results as required

Maintain accurate clinical records in conjunction with NHS guidelines for good practice
