Job responsibilities

You will work as an autonomous practitioner evaluating patients with complex care needs by using physical assessment skills, independent nurse prescribing (V300) Assess, create or update personalised care plans for vulnerable patients living out local primary care network area of Rye, Camber, Winchelsea Sedlescombe, Westfield Northiam, Broad oak Robertsbridge and BurwashTriage and manage acute and non- complex medical needs of patients who are housebound, palliative care and vulnerable, within your scope of practice and knowledge. Where other clinical support is, needed refer to GP or seek appropriate support of other community basis services.In conjunction with our care coordinators, work with other local service providers such as GP surgeries, community services, secondary care and voluntary sector to ensure that there is good communication between the multi-disciplinary services.Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice or support patients to access care for these issuesImplement and evaluate individual specialised treatment plans for chronic disease patientsIdentify, manage and support patients at risk of developing long-term conditions with the aim of preventing and managing the adverse effects these diseases may have on the patients healthProvide advanced, specialist nursing care to patients as required in accordance with evidence based evidence, NICE and the NSFSupport the undertaking of simple diagnostic procedures such:-Undertaking of ECGs and blood pressure monitoringUndertaking observations including monitoring for sepsisUndertake the collection of pathological specimensRequest pathology services as necessaryProcess and interpret pathology and other test results as requiredMaintain accurate clinical records in conjunction with NHS guidelines for good practiceYou will be at the heart of our primary care network program to support vulnerable patients. Proactively working with practices, other voluntary agencies and our care coordinators to identify and care for vulnerable patients in care homes and the local community. Such as those who have complex medical issues, palliative care patients, those who have Learning disabilities, Autism , Serious mental health illness and those who struggle to access medical services directly from a GP practice and would benefit from support around managing their ongoing care. You will visit patients in their own homes and in care homes, review and assess their health and wellbeing needs, design and agree with the patients a personalised care plan. Then work with patients, their families, carers and the wider Multi-disciplinary team within community and secondary care to ensure that they can live the best possibly life within the boundaries of their medical limitations. Our aim is for you to work alongside key community services already in place to provide co-ordinated personalised care for those in care homes/housebound patients to prevent un-necessary hospital admissions.You will also offer support to our established e-hub for online consultations providing cover when needed for sickness/annual leave. You will be able to assess and manage patients presenting with a range of acute and chronic medical conditions such as acne, rashes, eczema, contraception, HRT, conjunctavitis, sore throats, urine infections, pain management, colds and respiratory issues.

• Recognise, assess and refer patients presenting with mental health needs.
• Support patients in the use of prescribed medicines or over the counter medicines (within a scope of practice, reviewing annually as required)
• Maintain accurate clinical records in conjunction with current legislation
• Ensure read codes are used effectively.

Person Specification

Qualifications

Essential

• Advanced Practice Qualification (Masters Level) or equivalent experience (BSc Nurse Practitioner/Community Specialist Practitioner) and have completed a level 7 module which will give you access to the e-portfolio route of registration as an Advanced Nurse Practitioner (Health Education England requirement) which is an expectation of this role, Experience of working in a primary care /Urgent Care/A&E Experience of working as a GP practice or relevant similar experience. Experience of carrying out through medical assessment of patients and creating treatment plan of their needs Experience of prescribing and undertaking medication reviews or similar relevant experience.
• Qualified Independent Nurse Prescriber V300 essential for this role.

Desirable

• Qualified Triage assessment practitioner or other relevant experience Minor Illness Qualification- or willingness to undertake training Mentoring and supervision training undertaken.
• Experience of working with patients with long term conditions/frailty
