Job summary
We are looking for
an experienced, enthusiastic, friendly and professional Specialist Diabetes Practice Nurse to join our team at
Pontesbury & Worthen Medical Practice on a permanent contract basis. Applicants must have experience of working in a GP Practice.
The successful
applicant would be working alongside our current nursing team, and be responsible for the delivery
of Diabetes practice nursing services to our patient population.
Main duties of the job
Your main responsibilities will be the routine evidence based care of our diabetic population, including annual reviews incorporating the local prescribing guidelines as well as following up patients with more complex diabetic needs. The ideal candidate would have current Primary Care experience and would be a non-medical prescriber but this is not essential.
