We are looking for a committed, enthusiastic person to join our innovative multi-disciplinary team based in York's newest purpose-built primary care facility.

As the successful candidate, you will be a key part of our two Partner practice and will work alongside a growing team.

You will be responsible for delivering healthcare to people who live and work in York, as well as staff and students at the University of York.

Unity Health currently operates two surgeries to the South and East of York, close to excellent local amenities. We are a consistently high QOF achiever, a supportive member of the Humber & North Yorkshire ICB and an active member of the Student Health Association (SHA).

Main duties of the job

Accountable for the delivery of efficient, quality and evidence based clinical care in accordance to professional standards, guidelines and code of conduct (Safeguarding, Nursing and Midwifery Council, NICE, Local Authority Guidelines)

Chronic Disease Management including but not limited to:

  • Asthma
  • Vascular Disease (including IHD/Stroke/PVD)
  • COPD
  • Diabetes
  • Hypertension

To undertake clinical assessment and treatment of patients attending the Practice.

Function in a dynamic way ready to adjust to the changing needs of the client group.

As a Practice Nurse, its the nurses responsibility to adhere to any guidelines, protocols or best practice associated with any such additional elements of their role.

Family Planning including cervical cytology screening.

Assist in the delivery of specific campaigns ie Flu/Shingles.

Delivery of routine childhood vaccinations

Maintain stock of drugs and equipment.

Advise nurse management of equipment failure/maintenance requirements.

Contribute to QOF data collation as required.

Record all Patient Contact on Practice Medical Record system in a timely manner.

  • Conduct New Patient Health Checks.
  • Wound Management.
  • Spirometry

Perform monitoring of:

  • Patients for whom we provide long term contraception
  • Hypertensives

Perform wound care.

Perform urine dip tests

Review and amendment of nurse protocols as required
