Northlands Surgery has an exciting opportunity for a
Paramedic Practitioner to join our innovative and dynamic team.
We are a well-established, forward-thinking practice caring for a population of 14,000 patients. You will be joining an enthusiastic multidisciplinary team that includes GPs, Paramedic Practitioners, Pharmacy Technicians, Clinical Pharmacists, Practice Nurses, Healthcare Assistants and a highly skilled administrative team.

Main duties of the job
We are seeking a Paramedic Practitioner to primarily support our housebound patient population. You will receive clinical supervision and ongoing training opportunities to further develop your skills and confidence within Primary Care.
In this role, you will work closely with the practices clinical team and will benefit from excellent support, mentorship and regular supervision from a General Practitioner.
You will work autonomously in the community, delivering a range of services including assessment, diagnosis, treatment planning, telephone triage, clinical decision-making and appropriate referral under supervision.
The position is predominantly community-based and will involve undertaking home visits, liaising with the wider multidisciplinary team, and collaborating with other services to ensure patients receive the right care at the right time. The working day will start at the practice before carrying out visits.
The post holder will work within their scope of competence and report to a GP/Partner who will provide ongoing guidance, support and development in the role.

The Paramedic Practitioner will work as part of the multidisciplinary team at Northlands Surgery to provide high-quality, patient-centred care, with a primary focus on supporting housebound patients. The role involves autonomous working within the community, undertaking clinical assessments, formulating management plans, and contributing to safe and effective patient care.
The post holder will:

Provide comprehensive assessment, diagnosis and management of patients in their own homes, working within scope of practice and with GP supervision.

Deliver both urgent and routine home visits for housebound patients, escalating care when appropriate.

Undertake clinical duties including history taking, physical examination, clinical decision-making, treatment planning, and appropriate referral to other services.

Support telephone triage and remote consultations as required.

Work closely with GPs, nurses, pharmacists and the wider multidisciplinary team to ensure patients receive timely and appropriate care.

Liaise with community services, social care and external agencies involved in patient support.

Maintain accurate and contemporaneous clinical records using SystmOne in line with professional and practice standards.
