As a Nursing Team Care Coordinator, you will provide an important
role in supporting the nursing team alongside the wider practice and PCN team
with the delivery of high-quality primary healthcare.
Care coordinators play an important role within a PCN to
proactively identify and work with people, including the frail/elderly and
those with long-term conditions, to provide coordination and navigation of care
and support across health and care services.
The care coordinator role will ensure patient health and care
planning is timely, efficient, and patient-centred. The role will include
responsibilities for the coordination of the patients journey through primary
care and secondary care. This is achieved by bringing together all the
information about a persons identified care and support needs and exploring
options to meet these within a single personalised care and support plan, based
on what matters to the person.
The role will also assist the nursing team with their day-to-day
responsibilities by undertaking a variety of administrative duties. Some of
these duties include dealing with general queries via phone, email and post),
photocopying, scanning and filing.
Main duties of the job

- Plan the Asthma, COPD, Shingles and Pneumonia clinics. This includes reviewing recall lists, contacting patients via text, assessing which clinician is most suitable and booking appointments. Complete review forms and post management plans where necessary.
- Plan and book the cancer care assessments & reviews with the Practice Nurse Team
- Plan the Covid Clinics, this includes reviewing the national booking system, liaising to ensure sufficient stock level of vaccines and arranging the correct number of staff to attend the clinic.
- Assist with the administration of the Annual Flu Campaign. This includes updating the practice flu plan, organising staff for clinics, arranging visits to care homes and housebound patients, distributing letters to patients and other healthcare services, logging and communicating Patient Group Directions (PGDs) and Patient Specific Directions (PSDs).
- Monitor stock levels, and order Immunisations and vaccines where necessary, and in adherence to protocol.
- Monitor dressings stock levels, and order where necessary in adherence with the protocol.
- Support Bradstow School in the management of Immunisations and vaccinations for children. This includes keeping track of any outstanding and required Immunisations/vaccines, liaising with the school for consent forms and, arranging nurse visits.

- Miscellaneous
- Engage with patients to assist them in managing their healthcare, answering queries where appropriate.

- Receive any deliveries of onsite drugs, ensure secure and safe storage and review daily stock to ensure all are accounted for.
