Job summary
We are looking to develop and enhance our clinical skills mix in the
practice to complement and strengthen our existing team of experienced GPs, GP
Nurses, HCAs, Clinical Pharmacists and GPSTs. We are looking for 37.5 to 75 paramedic
hours per week, covered by 1-2 individual paramedics, according to applicants existing
experience base and skill sets
Main duties of the job
We are predominantly looking for practice-based working
with an initial focus on face-to-face Minor and Acute Illness / Injury
assessment and management. We are also looking for care-home clinical
management sessions which will be primarily based in our 3 practice-linked
residential care homes. We will support skills development in this area. Practice-based
work in record-keeping, data recording and coding in these areas of
responsibility will be important, with supported skills development in this.
Home Visiting opportunities for non-care-home residents will exist, although we
have a traditionally low home-visiting rate and long-established access to an
ICB-commissioned In-Hours GP visiting service. Care-home work will be conducted
with help and supervision from a named care-home lead GP, and practice-based
patient contacts will be supported by access to a named supervising GP for each
session worked. We feel this predominantly practice-based work pattern will
also help our new Paramedic(s) with rapid integration into the team and in
helping them to develop their skills in a supportive environment for
professional development. Our daily GP timetable currently includes one
supervising GP without booked patients working every session to help in
clinical and educational supervision of our GP Trainees and all clinical staff.
About us
Our current GP cohort comprises 6 GP partners, 6 salaried
GPs and 6 GPSTs (GP Trainees). Our experienced team of GPNs and HCAs now
numbers 5, with an excellent spread of Chronic Disease Management, general
nursing and developing Minor Illness skills. We benefit from an excellent
Clinical Pharmacist. We work with 5 fellow practices in our PCN (Icknield),
based around Baldock, Letchworth and Ashwell. Our PCN is a friendly, cohesive
and supportive group. Our newly appointed PCN Training team is led by one of
our GPs and GPNs. This team includes specific and dedicated AHP support, which
would cover our new paramedic(s). This team is supported by Hertfordshire &
West Essex ICB and Training Hub. One of our GPs also works part-time with the
Training Hub in a Training Expansion Lead role.

- The role will provide a Paramedic Practitioner resource for health care professionals and service users, working in collaboration with other members of the Multidisciplinary Team
- To work within the practice and community, as an autonomous, accountable Paramedic Practitioner in the provision of holistic care for patients including assessment, diagnosis and treatment, and to deliver quality patient services
- Support the delivery of home visits to care home and housebound patients. The practice context is of a historical and current low visiting rate to non-care home patients, which is expected to continue
- Support the delivery of minor and acute illness care to patients from the practice base, predominantly for face-to-face contacts, but potentially for some remote contacts as the role develops
- To assess, diagnose, treat, refer or signpost patients who present with undifferentiated or undiagnosed conditions relating to minor illness, acute illness or minor injury
- The post holder will use advanced clinical skills to provide education to patients, promoting self-care, and empower them to make informed choices about their treatment
- The post holder must own a vehicle and driving licence for home visits with expenses remunerated in the overall financial package

- Treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team. Advanced skills or skills development may involve / progress to some triaging of patients
- Refer patients as required to secondary care. This may require discussion with the registered, supervising or other supporting GP in some cases before making such a referral
- Ensure clinical practice is safe and effective and remains within boundaries of competence, and acknowledging of limitations
- Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary
- Work from the surgery and within communities as an autonomous practitioner caring for patients and their families
- Work as an autonomous practitioner, in accordance with the Health Professions Council. Ensure that personal and professional clinical standards are maintained
- To undertake assessment for patients within the community and those attending the surgery, using diagnostic skills and initiation of investigations where appropriate
- Prescribe/issue medications as appropriate following policy, patient group directives and local pathways, and according to personal competence and skillsets for non-medical prescriber paramedics. Development of non-medical prescribing qualification in those not already qualified will be encouraged
- To impart knowledge and skills formally and informally to colleagues, promoting peer review and best practice within the work environment
- To communicate at all levels within the team ensuring an effective service is delivered
- To maintain accurate, contemporaneous healthcare records appropriate to the consultation
- Ensure evidenced-based care is delivered at the highest standards, ensuring delivery of high-quality patient care
