An exciting opportunity has
arisen for a proactive, patient-centred Occupational Therapist to join a small
team of OTs within East Lindsey Primary Care Network.
We are looking for 3 Part-time OTs to cover, North Thoresby/Binbrook, Caistor and New Coningsby.
Occupational Therapy is at the
core of supporting patients to live well, maintain function and recover from
ill health and our Occupational Therapy team will be an integral part of
improving the wellbeing of our patients in the NHS Long term plan and the
developing Lincolnshire Frailty Strategy.
The successful candidate will
be required to utilise their in-depth knowledge, clinical skills and experience
at a senior level, in the management of patients Occupational Therapy
conditions within the remit of falls and frailty. We are focused on early
intervention with a goal to support patients proactively, to avoid falls and
prevent or slow down frailty progression. Patients will be seen in a variety of
settings including GP surgeries, care homes and domiciliary settings.
The post holder will work collaboratively with the PCN and surgery based multi-disciplinary
teams (MDT) to meet the needs of patients and carers, support the delivery of
policy and procedures, and provide Occupational Therapy education as required.
Main duties of the job
Holistic assessment of and intervention for patients
experiencing or at risk of frailty and falls in their own homes, GP surgeries
and care homes.
Ensure all assessments and interventions are patient-centred,
clinically effective and of sound evidence-base.

Ability to manage a caseload, work independently as well as
part of a multi-disciplinary team to provide specialist, evidence-based,
holistic assessment and intervention to patients.

Ability to motivate patients to engage in self-management of
their health conditions
help patients to adapt to and manage their physical and
mental health long-term conditions, through the teaching of coping strategies

Therapists cover single practices to allow you to fully integrate into that
practice to be an integral part of the practice MDT.
Since the inception of Primary
Care Networks, we have worked proactively to provide Occupational Therapists, Clinical
Pharmacists, Pharmacy Technicians, Social Prescribers, First Contact
Physiotherapists, Paramedics, Mental Health workers and care coordinators to
optimise the services available to our population and created a robust and
diverse skill set across our PCN. We continue to forge relationships to support
the future development of our PCN and our workforce.

Possess good communication skills in both verbal and written
forms to allow effective MDT working and communication with patients and their
carers, considering alternative modes of communication where appropriate.
