The successful candidate will play a key role in delivering high-quality nursing care, supporting patients with long-term conditions, and contributing to health promotion and disease prevention. You will manage and review patients with chronic diseases such as diabetes, asthma, COPD and hypertension, and carry out cervical cytology, childhood and adult immunisations, travel vaccinations and phlebotomy. You will also contribute to QOF, IIF and enhanced service targets, working closely with GPs, ANPs, HCAs and pharmacists.

We are looking for an experienced, motivated nurse who can work independently and collaboratively, with excellent communication skills and a commitment to quality improvement and continuous learning. Experience in primary care and use of EMIS Web or Ardens templates would be an advantage.

Main duties of the job

The Practice Nurse will provide high-quality, evidence-based nursing care to patients at Chorlton Family Practice. This includes managing patients with long-term conditions such as asthma, COPD, diabetes, and hypertension, undertaking chronic disease reviews, and supporting medication management. The role also involves performing cervical cytology, administering childhood and adult immunisations, travel vaccinations, phlebotomy, and providing health promotion and lifestyle advice. You will contribute to the delivery of QOF, IIF, and enhanced services, ensuring accurate clinical documentation using EMIS Web and Ardens templates. Working collaboratively with GPs, ANPs, HCAs, and the wider clinical team, you will support safe, effective, and patient-centred care while maintaining infection control and safeguarding standards.

Job Purpose

To provide high-quality, evidence-based nursing care to patients registered at Chorlton Family Practice. This includes long-term condition management, treatment room duties, health promotion, immunisations, and supporting the delivery of QOF, IIF, and enhanced services. The role requires working collaboratively with the wider clinical and administrative team to ensure excellent patient outcomes and a safe, effective service.

Main Duties and Responsibilities1. Clinical Care
  • Manage and provide care for patients with long-term conditions such as diabetes, asthma, COPD, and hypertension.

  • Undertake chronic disease reviews, medication checks, and titration in accordance with local and national guidelines.

  • Perform cervical cytology, childhood and adult immunisations, and travel vaccinations.

  • Carry out treatment room duties including phlebotomy and administration of therapeutic injections (e.g. B12, Zoladex, Prostap).

  • Provide health promotion and lifestyle advice on smoking cessation, weight management, diet, and exercise.

  • Support womens and mens health services as required.

  • Maintain high standards of infection prevention and control in all aspects of clinical practice.

2. Health Promotion and Public Health
  • Deliver patient education to promote self-care and healthy lifestyle choices.

  • Participate in national and local health campaigns (e.g. flu, NHS Health Checks).

  • Identify and respond to safeguarding concerns for both adults and children in line with practice and statutory procedures.

3. Teamwork and Communication
