Job summary

We are looking for an Experienced Practice Nurse to join our forward thinking Practice Team.

We are willing to consider Part or full time and can offer flexibility of start and finish times for the suitable applicant.

Our nursing team is well supported and has regular team meetings

About us

We are a 2 site practice based in Hillsborough and Stannington. We have 4 GP Partners and 3 Salaried GP's. Our nursing team consists of ANP/Manager, 1 ANP, 2 Practice Nurses and 3 Health Care Assistants. Our pharmacy has 2 pharmacists and 1 Pharmacy/ACP trainee.

We are part of the Hillsborough PCN which consists of the 4 local practices and we share a variety of services Occupational Therapist, Mental Health Team Additional GP and ANP services.

As a PCN we cover our own Extend Access, this means each Practice covers one evening and 1:4 Friday/Saturdays

Main Purpose of Job:

To work as part of the primary Health Care Team in providing and maintaining a high standard of nursing care to the practice population and work within the scope of the NMC code of professional conduct.

This job description outlines the duties as currently required but may be amended to reflect future developments in the service and the impact of new technology on the role. Appropriate training will be provided to support essential skills required.

Job responsibilities

Clinical

  • General duties of a Practice Nurse
  • Chronic Disease Reviews

  • To provide appropriate treatment to the Practice population in accordance with Practice protocols and procedures and adhere to Patient Group Directives
  • Ensure infection control guidelines are maintained
  • Maintain accurate records and document consultations according to local and NMC guidelines
  • Ensure awareness of statutory and local clinical protection procedures, including systems of referral including child protection strategies
  • To liaise with other members of the primary Health Care team as required

Educational

  • Participate in continuing education as identified either personally or by the Practice to maintain a high level of professional knowledge and skills
Personal/Professional Development

  • Identify personal development and training needs in conjunction with the Nurse Team Manager
  • Participate in annual appraisal, taking responsibility for maintaining a record of personal and/or professional developments (PDP) for revalidation purposes
  • Take responsibility for own development, learning and performance and demonstrate skills and activities to other team members who undertake similar work
