Job summary

An exciting opportunity has arisen for a part-time Advanced Nurses/Clinical Practitioner to join our expanding, friendly, forward-thinking team at Severn Valley Medical Practice. We are looking for 30 hours per week, to be delivered across 4 days.

The successful candidate will deliver a high standard of autonomous clinical care through advanced assessment, diagnosis and treatment. As an independent prescriber, you will work as part of a multi-disciplinary team supporting patients requiring urgent/same day care.

We are a pro-active member practice of Worcester City Primary Care Network that provides services to some 130,000 patients. The strength of our PCN lies in its size and its robust leadership.

Main duties of the job

As an Advanced Clinical/Nurse Practitioner, you will provide high-quality, patient centred care within a busy GP practice setting. Acting within your professional boundaries, you will manage patients presenting with a range of acute and chronic conditions from initial triage, consultation, through to diagnosis and treatment.

This role combines autonomous clinical practice with collaborative teamworking.

  • Provide clinical assessment, diagnosis and treatment to patients both face-to-face, remotely and in patient homes.
  • Manage both acute and chronic conditions within your professional boundaries autonomously.
  • As an independent prescriber, you will prescribe medications appropriately and safely.
  • Request, interpret and act on diagnostic tests and investigations.
  • Refer patients to appropriate services.

About us

Severn Valley Medical Practice is a dual sited practice, with practices at Henwick Halt and Lyppard Grange. Both are purpose built as medical practices and have ample staff and patient parking.

Our raw list size is currently 21,800 patients. We boast 17 GP's, 5 ANP's and a substantial nursing team that includes HCA and phlebotomy services. Both sites benefit from a strong non-clinical workforce to support patient access, communications and services.

The practice uses EMIS Web clinical system. We have an embedded legacy of medical education and proactively support employee development and advancement wherever possible. Severn Valley Medical Practice mentored the first Primary Care ANP in Worcester. As a teaching practice we have worked with Birmingham University for over 30 years, as GP trainers for 25 years and played an integral part in establishing Worcester University Medical School.
