Job summary
We are looking for an enthusiastic, motivated experienced
general Practice Pharmacist who is committed to developing their knowledge and
skills to improve patient care.
The Practice-based Clinical Pharmacist will improve
the quality and safety of care offered to patients of Harford Health Centre.
They will have regular contact with patients and will reduce the workload of GP
prescribers in the practice by performing structured medication reviews. They
will also support the management of patients with long-term conditions by
reviewing repeat medication requests.
They will co-ordinate prescribers at the practice
to ensure that appropriate medication is prescribed, ensuring that income and
dispensary profit is maintained whilst fulfilling the requirements of CCG and
locality initiatives.
Main duties of the job
The Clinical Pharmacist will develop
and manage medicines optimisation services within primary care.
The key aims for the clinical
pharmacist are:
To reduce the medicines related
workload on doctors
Increase safety and quality of
prescribed medicines
Provide support on medicines related
issues to the whole practice team
Be a point of contact for patients
regarding medicines queries
A senior advisory role in the day to
day running of the dispensary
They should support the following
clinical work:
Clinical Medication Review
Chronic Disease Management (for
example, managing hypertension/Type 2 Diabetes/Asthma/COPD patients,
polypharmacy review)
Use appropriate CCG provided clinical
templates within the EMIS Web Clinical System
Medicines reconciliation on transfer
between care providers
They should also support the
following practice development work:
Focusing on QoF Domains, improving
disease registers
Prescribing Safety, Clinical audit,
Local Enhanced Services support
Lead on prescription management
(systems and processes, ensuring monitoring is being done, supporting
dispensary & clerical (repeat prescription) staff)

Harford Health Centre is located in a
purpose built health centre building. This is an exciting opportunity to join a
forward thinking modern General Practice. We are a busy dispensing practice,
dispensing around 11,000 items per month. We have a list size of 10000 patients
which will be increasing over the coming years as new residences are built in
our locality.

systems for safer prescribing including Eclipse. They will perform face-to-face
medication reviews ofpatients with polypharmacy;
especially for those with frailty and/or with multiple co-morbidities. They
will undertake face-to-face reviews of patients with specific long term
conditions that fall within their competency. They will provide leadership on quality
improvement and clinical audit, as well as managing aspects of medicines safety
and antibiotic stewardship.

- Masters degree in Pharmacy (MPharm) or equivalent
- Registered with GPhC
- Member of RPS
Desirable
- Postgraduate knowledge through a Postgraduate Diploma and qualifications (e.g. clinical, community, therapeutics) or equivalent.
- Independent prescriber status
Experience

- Additional qualification in chronic disease or minor ailments would be advantageous.
- Experience in medication reviews and basic clinical skills.
- A community or CCG background who have experience with drug budgets and switches.
