Little
Park Surgery has partnered with Hounslow Public Health to develop an innovative
role that bridges proactive community outreach with direct integration of
clinical information into GP recordsensuring that insights gathered in the
community setting are captured, shared, and translated into more responsive,
personalised patient care.
The
Health Outreach Clinical Support Officer is a key member of the Hounslow Health
Outreach Team, supporting the mission to reduce health inequalities by engaging
residents directly within their communities.
This role
combines community outreach with a primary focus on accurate clinical data
recording. You will ensure all resident interactions contribute to safe,
continuous, and proactive care by maintaining high-quality clinical records and
supporting effective communication with GP practices.

Key Responsibilities
1. Data Recording, Clinical Coding & GP
Reporting (Primary Function)
-Act as the lead officer
responsible for accurate and consistent data entry into SystmOne.
- Collect and input assessment
data from Health Outreach Officers, including blood pressure (BP), BMI, CO
readings, gait and balance results, and intervention notes.
- Ensure all readings and
referral SNOMED codes are recorded safely and in line with Primary Care
policies.
- Report relevant findings to
GP practices to support clinical oversight and follow-up care.
- Maintain high standards of
accuracy, confidentiality, and clinical governance in all data handling.
2. Community Outreach & Resident Engagement
(Supporting Function)
-Support outreach sessions by
conducting brief health assessments when required.
- Engage residents in
meaningful conversations to understand their health needs.
- Use motivational
interviewing techniques to provide guidance and signposting to appropriate
services.
- Contribute to increasing
awareness and uptake of local health and wellbeing services.
- Accurately document resident
insights and interactions.
3. Supporting Residents & Coordinating Care
-Help residents identify
factors affecting their health and wellbeing.
- Input referral information
into SystmOne for services such as mental health, falls prevention, social
prescribing, and stop-smoking support.
- Record personalised care and
support plans to ensure continuity of care.
- Provide appropriate
next-step guidance based on health assessments (where clinically
appropriate).
4. Teamwork, Communication & Collaboration
-Work closely with Health
Outreach Officers to collect daily assessment data.
- Communicate effectively with
team members and escalate issues when necessary.
- Promote a culture where
every contact counts, ensuring all interactions are accurately recorded.
- Provide feedback and
insights to support service improvement.
5. Operational & Practical Responsibilities
-Work flexibly across a
seven-day rota to support outreach delivery.
- Participate in outdoor,
community-based work, including standing and walking for extended periods.
- Maintain safe, clean, and
professional outreach environments (mobile units and fixed sites).
- Follow operational protocols
and use resources responsibly.
- Promote consistent public
health messaging and positive health behaviours.

-Deliver a proactive, approachable,
and resident-focused service.

non-judgemental, inclusive, and strengths-based way.
