Chesser Surgery is seeking a motivated Practice Nurse to join our team and provide high-quality care to patients.

We are looking for a qualified General Practice Nurse.

This is apart-time role (16 hours per week)but we are open to discussing flexible arrangements to suit the right candidate.

Main duties of the job

Main duties of the job

As a Practice Nurse, you will:

  • Conduct comprehensive assessments of patients health needs.

  • Develop, implement, and review individualised care plans.

  • Provide appropriate treatments and interventions as required.

  • Undertake treatment room responsibilities and deliver Chronic Disease Management clinics.

  • Carry out specific clinical tasks, including but not limited to :

    • Travel health consultations and vaccinations

    • Cervical screening tests

    • Childhood immunisations

    • Wound management

    • Blood pressure monitoring and ECGs

    • Venepuncture and health checks

  • Work collaboratively with GPs, other nurses, and the wider healthcare team to ensure high-quality patient care.

  • Maintain accurate, timely, and confidential patient records in line with NHS standards.

  • Deliver health education, advice, and counselling to patients and their families.

  • Keep up to date with current clinical guidelines, medical advancements, and best practice in primary care.

  • -Evidence of current NMC revalidation
  • -Experience in primary care or transferable clinical setting
  • -Competence in immunisations and vaccinations (adult and childhood)
  • -Cervical cytology training
  • -Venepuncture skills
  • -Wound care and dressings experience
  • -Ability to manage Chronic Disease Management (CDM) clinics (e.g., diabetes, asthma, COPD, hypertension)
  • -Good IT skills and ability to use electronic patient records

  • Previous experience working as a Practice Nurse in General Practice
