Job summary
The PCN Assistant
supports the PCN management team and East Cambs Integrated Neighbourhood team with coordinating, organising and checking progress on various PCN/Neighbourhood
contract requirements and projects.
The PCN Assistant plays
a crucial role in the smooth running of the PCN and ensuring tasks are completed
on time. Responsibilities include liaising with practice and Neighbourhood colleagues, preparing
reports/data, tracking key performance indicators, and facilitating
communication between different stakeholders.
(Interviews will be held on Thursday 14th August.)

About us
Ely PCNs is a
network of seven GP Practices in East Cambridgeshire, which together look after
86,000 patients. We support an additional 50 staff, including Social
Prescribers, First Contact Physios, Care Coordinators, Health and Wellbeing
Coaches, Pharmacy Technicians, and Clinical Pharmacists.
Ely PCNs has a
full-time manager, a Lead Nurse, and GP Leads for Diabetes, Frailty and Mental
Health. We have two forward-thinking Clinical Directors and excellent
relationships with our local partner organisations.
We have a
reputation as an innovative PCN that can successfully deliver. Initiatives we
support include an infant feeding support project for new parents, a project
for young people who have experienced domestic violence, and a rapid-response
substance misuse worker, and a community heart failure clinic, as well as
facilitating an Enhanced Access out of hours service and the local Covid
vaccination programmes.

comfortable using IT. Problem-solving, pragmatism, and organisation are
essential to the success of this role, as is the ability to build professional
relationships with colleagues across Ely PCNs seven practices.

Support the management team to meet National Network
DES contract requirements.

Coordinate our monthly Frailty MDT in relation
to Enhanced Health in Care Homes and minute any actions required.

Collect and input data into Clinical System
(SystmOne) when requested.

Support the management team with National capacity
and access requirements and submissions.

Support the PCN Enhanced Access manager with
day-to-day operations, including rota management.

Support the management team with coordination
and admin associated with new and ongoing PCN projects.

Support the management team with PCN finances including
invoicing and recording as necessary in PCN Xero accounting system.

Support the management team with diary management
and booking meeting rooms and venues for events as required.

Support with the coordination of staff annual
appraisals.
