Job summary
We have an exiting opportunity for an Advanced Nurse Practitioner to work part time at our surgery two to three days a week.
You will support the GP team and Nurses/HCAs managing same day appointment requests from referred patients.
The role will involve independent prescribing.
Experience in Diabetes management would be desirable but not essential.
Main duties of the job
You will work independently, making professional, autonomous decisions.
The role will involve initial history taking, clinical assessment, diagnosis, treatment plan and evaluation of patient care. Prescribing and referring to secondary care, where appropriate.
You will be responsible for making any necessary referrals, requests for investigations and do these in in an appropriate manner and timescale.
The nurse practitioner will need to prioritise and triage the needs of patients, accordingly, seeking GP input as appropriate where the problem lies outside their professional capability.
You will demonstrate safe clinical decision making and a high level of independent care including assessment, diagnostic and prescribing skills. You may need to attend patients with urgent medical problems.

Goldsworth Medical Practice is a high performing forward thinking and friendly GP training practice with a strong ethos of continuity of patient care, team work and peer support.
We have a list size of circa 13100 patients, use EMIS and our most recent CQC rating was GOOD. We operate on a mixture of clinical triage, multidisciplinary working and customised sessions.
Our practice employs a wide MDT that shares the GP workload and currently comprises our experienced GP Partners, Salaried GPs, Advanced Nurse Practitioner, Chronic Disease Specialist Nurses, Practice Nurses, HCA's, Paramedic, Physician Associates and Clinical Pharmacists who work across both of our sites.

Main duties of the job
To work as an autonomous practitioner
- Attending to patients who wish to be seen by a health care professional / phone consultations / triage.
- Diagnosing, treating, and prescribing for patients presenting with undifferentiated and undiagnosed illness or injury.
- Assessing patients needs, based on highly developed nursing skills sometimes including physical assessment.
- Overseeing and advising people in respect of their continuing medical and nursing needs.
- Patients are to be seen in a timely manner enabling contractual requirements to be met.
- The ANP will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
- As a clinician be available to support other healthcare team members and to accept referrals from other team members.
- To liaise with all members of the Primary Health Care Team and other agencies local authority, social services, secondary care, voluntary sector, and Primary Care in order to assure appropriate care is provided for the practice population.
- To participate as a key member of the multi-professional team through the development of collaborative and innovative practice.
Prescriber
- To have clinical and professional accountability for prescribing practice, working at all times within the practitioners clinical competence and with reference to the NMC professional standards
- To prescribe safe, evidence based, cost-effective and appropriate medication as defined by current legislative framework and within the local non-medical prescribing policy
- To prescribe within the prescribing formulary according to agreed frameworks of prescribing competency
- To follow agreed national and local treatment guidelines, ensuring all care is delivered within the practitioners professional scope of practice
- Offer holistic service to patients, developing where appropriate an ongoing plan of care/support, with an emphasis on prevention and self-care
- Contribute to the GPN aims of sustaining high standards of patient care and service delivery
- To record consultation details and medication prescribed in the patient notes contemporaneously in accordance with NMC guidance on record keeping
- To ensure notification to the practice/CCG if prescribing duties cease or change
