Main duties of the job

  • Provide safe, evidence-based primary care: acute presentations, long-term condition reviews, health promotion and screening.

  • Undertake a mix of face-to-face and telephone consultations; triage and same-day demand as part of the duty rota.

  • Prescribe and deprescribe in line with local formulary; complete medicines reviews and safety-net appropriately.

  • Request, interpret and act on investigations; review results, correspondence and clinical workflows promptly.

  • Make referrals via appropriate pathways; coordinate care with community, secondary care and PCN services.

  • Maintain accurate, coded clinical records; complete fit notes, reports and safeguarding documentation.

  • Contribute to QOF, IIF/PCN DES and Enhanced Services delivery, including call/recall where required

  • Engage in CPD, appraisal and revalidation; comply with GMC, CQC, NHS and practice policies.

Care home ward round

  • Conduct regular, structured ward rounds for allocated care home(s), including urgent reviews when needed as part of teh care home team (1 ward round a week).

  • Manage anticipatory care and end-of-life planning (including ReSPECT/DNACPR) and symptom control.

  • Work MDT-style with care home staff, community nursing, AHPs, pharmacy and palliative teams; ensure clear safety-netting and escalation plans.

  • Record contemporaneous notes, code activity, and contribute to EHCH reporting and outcomes.

We are an enthusiastic and forward-thinking team of health care professionals and administrative colleagues, committed to multi-disciplinary team working and offering patient centered high quality care.

We use total a "Total triage" or "Modern general practice" pathway which ensure that all patient medical request are reviewed by a clinician, prior to appointments being offered. This means that we can ensure the patients are seen by the appropriate service.

The Practice uses Accurx and Docman for Workflow management. We also outsource medical report requests.

GP led long term condition clinics.

High QOF achievement

CQC rating of good across all areas.

Strong links with PRIME PCN, Wandsworth GP federation and Wandsworth ICB team.
