Experience within Primary Care is essential for this role, as the successful candidates must have an understanding of patient health services and how they are successfully managed within local GP practices, together with a passion for improving lives and improving healthcare.
You will be leading on regular and ad hoc projects, particularly relating to our Primary Care Network (PCN) objectives.
Main duties of the job
To be responsible for undertaking a wide range of administrative duties and the provision of administrative support to the multidisciplinary team. Duties can include but are not limited to supporting the practice in its PCN schemes such as Enhanced Services (e.g. Access, Diabetes, SMI, Influenza/Covid vaccinations, immunisation, LD checks etc.), Investment and Impact Fund (IIF) in addition to a variety of regular and ad hoc projects. This will include assisting in the management of clinical correspondence, ensuring it is allocated to the correct team.
We require a Care Coordinator to join our team to provide and maintain high standards of patient care as well as administrative tasks. The successful candidate will enjoy working as part of a dedicated multidisciplinary team, with the added satisfaction of working for an organisation committed to ensuring all our patients receive an excellent level of care.
Specific areas of work include supporting General Practices and the Primary Care Network to deliver the Local Enhanced Services, which includes successfully supporting the clinical team in the management of Long Term Conditions. This is to ensure patients with specific conditions are successfully managed and self-managed
