Training: we engage
with medical student placements and nurse mentoring. Although

We are a practice based 3 miles South East of Sheffield. the community of Sheffield isknown for its excellent schools and the scenic
beauty of nearby Derbyshire, making it a wonderful place for both personal and

The practice serves a population of around 8750 patients with the area being identified as having high levels of deprivation. It is one of the more deprived areas nationally in terms of socioeconomic indicators.

We are a key practice in the Heeley Plus PCN, we are at the forefront of collaborative healthcare delivery. We work closely with social prescribers and have exciting plans to integrate further with wrap-around services like Health Trainers and Link Workers. This holistic approach allows us to provide comprehensive care that addresses the broader health and wellbeing needs of our patients.

1. Patient Care and Clinical Management
- Diagnose, treat, and manage a wide range of acute and chronic medical conditions.
- Conduct consultations through face-to-face, telephone, or online appointments.
- Provide preventive care, health education, and lifestyle advice.
- Prescribe medications safely and appropriately, following national and local guidelines.
- Deliver holistic care that considers physical, psychological, and social factors.
- Arrange investigations, interpret results, and follow up appropriately.

2. Continuity and Coordination of Care
- Maintain ongoing relationships with patients and families, often over many years.
- Coordinate multidisciplinary care by working closely with nurses, pharmacists, allied health professionals, and social services.
- Support patients in navigating the wider healthcare system, ensuring seamless care transitions.
3. Preventive and Population Health
- Participate in screening, immunisation, and health promotion programmes.
- Identify and manage risk factors for disease within the practice population.
- Contribute to local and national public health initiatives.
4. Quality and Governance
- Engage in clinical audit, quality improvement projects, and appraisal processes.
- Adhere to GMC standards and clinical governance frameworks.
- Ensure accurate and timely clinical documentation using electronic health records.
- Participate in safeguarding, infection control, and medicines management.
5. Education, Training, and Leadership
- Support the education of medical students, GP trainees, and other clinical staff (if applicable).
- Take part in continuing professional development (CPD) and annual revalidation.
- Contribute to service development, clinical leadership, and innovation within the practice or Primary Care Network (PCN).
