An exciting opportunity has arisen to join our friendly
and forward-thinking GP practice as a Health Care Assistant!
We're a welcoming team that prides itself on delivering
excellent patient care while fostering a supportive and enjoyable working
environment. With a patient list of around 8,500, you'll be part of a great
clinical and administrative team working under the supervision of the Lead Nurse, supporting the delivery of high-quality care to patients and contributing to the smooth running of the clinical services.

Main duties of the job
A key part of your role will be chronic disease management in monitoring long term conditions such as diabetes, asthma, and hypertension (under nurse/GP guidance).

About us
Our practice serves the northern part of the Gleadless Valley estate and supports a patient list of approximately 8500 individuals. It's acknowledged by the city council as one of Sheffield's most deprived areas so the role can be challenging but very rewarding at the same time.
The team not only work well together to provide the best care that they can but they also enjoy getting involved in social activities from local Park Runs to the occasional team bake-off (we take cake very seriously!).

Clinical Duties
- Perform routine clinical tasks including:
- Phlebotomy
- Blood pressure measurement
- Urinalysis and specimen collection
- ECGs
- Height, weight, and BMI recording
- Wound care / dressings (as per competency)
- Flu and other vaccinations (after training and accreditation)
- Assist with chronic disease management clinics (e.g., diabetes, hypertension, asthma)
- Conduct annual health reviews
- Monitor and maintain clinical equipment (e.g., cleaning, calibration)
Administrative Duties
- Accurately record patient data and observations in the clinical system, EMIS
- Assist with call and recall systems for routine health checks and vaccinations
- Support the management of stock and supplies in treatment rooms
- Assist with infection control processes and ensure adherence to cleaning protocols
- Support the implementation of health promotion activities
