Job summary

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by a Practice Nurse, and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training. An interest in Diabetes, Asthma, Spriometry and fitting coils would be advantageous.

Main duties of the job

Assist in and perform routine tasks related to patient care as required. Such tasks may include:

Asthma, Diabetes, Wound care/Ulcer care, Removal of sutures, Spirometry, Travel Clinic, Immunisations/Childhood Immunisations, Cervical cytology and contraception advice.

We are a small friendly rural GP practice with 5,500 patients, 2 partners, 5 GPs , 2 practice nurses, Paramedic, Nursing associate, 2 HCAs and a phlebotomist. We are currently looking for a practice nurse to join our strong cohesive team . Charlbury Medical Centre is a member of West Oxfordshires Rural West PCN.

We pride ourselves on our use of information technology and have a team of motivated and experienced administration and management staff. We are a GMS contract holder and use EMIS Web, EPS electronic prescribing and Docman, and we have a high QOF achievement and a GOOD CQC status.

Job Summary:

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. oChaperoning and assisting patients where appropriate who are being examined by another clinician

oAssisting GPs with minor surgery and coil fittings

oRequesting basic pathology tests, for example urine culture, swabs

oTravel Clinic, providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection.

oRoutine immunisations/Childhood immunisations

oCervical cytology and contraception advice.

oWell person clinics and health promotion programmes: holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise. Facilitating behaviour change.

oProvide advice and educational material to patients to promote their understanding

oEnter data appropriately

oObtain and document informed consent (either verbal or written) when appropriate.

oEnsure infection control guidelines are maintained.

Managerial:

Contribute a nursing perspective to the practice development plan.

Facilitate the development of patient services in line with trends in General Practice

Manage and organise individual patient consultations.

Attend staff meetings.

Support the practice clinical governance agenda.

Identify changes to clinical practice that are required to implement evidence-based guidelines.

Educational:

Identify personal development and training needs in conjunction with the Lead Nurse/GP.

Address personal development needs.

Support others with their training and development needs.

Participate in continuing education and maintain a contemporary level of professional knowledge and skills.

Offer innovative ways of working and opportunities to facilitate learning.

Personnel:

Take reasonable care of his/her own safety and that of other persons who may be affected by his/her act or omission.

Co-operate with the practice to ensure all members of the team adheres to statutory regulations/policies and codes of practice and departments safety rules.
