SWAGGA PCN is looking for an enthusiastic, patient focused and highly motivated Registered Nurse Associate, with a passion for Womens Health. SWAGGA PCN was one of the first PCNs to develop a Womens health hub (by enabling access to LARC across practices). Since the SWAGGA PCN hub launched in Aug 2020 we have seen a significant rise in LARC activity across the network. We are looking for a candidate with good administrative skills who has an excellent connection with patients and is enthusiastic about training in new clinical skills pertaining to womens health (such as smear taking and ring pessary fitting). You will work alongside our GP lead for Womens Health to help develop and expand the Womens Health Hub.

SWAGGA Primary Care Network (PCN) is a dynamic collaboration between sixteen practices across South Liverpool. We have a vision to improve patient care, tackle inequalities and provide outstanding support for our member practices. We plan to make a lasting impact upon the communities we serve and are developing ambitious plans for the future.

Main duties of the job

Our combined patient population is approximately 100,000 and we work very closely with our community team and other local Healthcare providers. This includes Salaried GP's, Advanced Clinical Practitioners, Clinical Pharmacists, Practice Nurses, Social Prescribers and First Contact Physios.

This is a full-time post (however candidates may enquire about part-time opportunities if called for interview).

The hours of the post will be based around 9am-5pm hours; however, post holders may be invited to participate in enhanced access clinics as additional optional over-time or with the option to receive time back during their usual job plan.

Your duties may include:

THE ROLE IN THE SWAGGA WOMENS HEALTH HUB:

To work closely with the Womens Health Lead to help develop the Womens Health Service. Your duties may include:

You will design and implement systems and processes to support the leadership team to ensure the PCN is able to identify the needs of women across the PCN and respond accordingly. The roles and responsibilities will be developed and supported alongside clinical competency frameworks (these will be agreed and assessed through senior nursing colleagues and with the support Womens Health Lead GP).

GENERAL NURSING ASSOCIATE RESPONSIBILITIES

To assist and perform routine nursing tasks related to patient care, such as blood pressure checks, routine diabetic foot checks, inhaler technique checks, immunisations, BMI lifestyle and advice, as directed and agreed with line manager, registered general nurses and GPs. To support patients to improve and maintain their mental, physical, behavioural health and well-being by using behaviour change interventions, such as undertaking health checks (physical and mental), smoking cessation services.

Following training and competency assessment, to undertake adult and child NHS immunisation programmes and ensure vaccines/injections are administrated under the appropriate legal framework. To undertake administration of prescribed medicines via IM, SC routes, following competency-based training and practice policy.

To support health action planning, as part of Learning Disability and Severe Mental Illness physical health checks, working closely with the network LD and SMI care coordinators, with the support of practice clinical leads and community specialist teams. To undertake the collection of pathological & microbiology specimens including venepuncture, swabs, urine culture in line with local guidelines. To support chronic disease management including monitoring patients with stable asthma, COPD and undertaking preliminary investigations for Type 2 diabetes patients and heart disease, such as urinalysis, arranging ECGs

To provide general health promotion education, advice on national screening programs and guide/refer patients to appropriate services. In line with national guidance and with recognised competency-based training, undertake screening programs such as cervical screening tests.

To undertake a basic history, order basic clinical investigations (such as urine for MC&S) and refer patients to relevant clinician for interpretation/ planning of care. To be able to provide appropriate self-care advice for patients presenting with minor illness within own level of competency and referring to GP/ANP where appropriate.

Chaperone and assist patients where appropriate who are being examined by another clinician. To maintain effective infection control measures in line with local guidelines and practice policy. To be able to recognise and manage anaphylaxis, according to the current UK Resuscitation Council guidelines.

To be able to perform Cardio-pulmonary resuscitation (BLS) and the correct use of Automated Defibrillator following training, according to the current UK Resuscitation Council guidelines. To engage in the support the training of nursing and medical students, and newly appointed colleagues. To participate in developments within the practice / network and support QOF / DES requirements.

To be registered with the NMC. Work in accordance with the NMC Code of Conduct and Scope of Practice for Nursing Associate. Ensure accurate notes of all consultations and treatments are recorded in the patients notes and on computers, as per NMC guidelines.

Attend and actively participate in practice and network meetings as required. Awareness of statutory protection procedures including referral and recognition of signs and symptoms of child abuse.

Awareness of referral mechanisms for mental health issues, family violence, vulnerable adults and addictive behaviours. Actively take part in mandatory and internalexternal training as requested. Adhere to and follow all practice policies and protocols.

Stakeholder Relationships

Work as part of the wider holistic team to provide cover and support as necessary. To work with our clinical and digital system colleagues to implement and operate technology solutions which may include wearables or equipment to enable self-taking of health diagnostics e.g., blood pressure, weight etc.

Build and maintain relationships with primary, secondary, community and voluntary leads and members of the local support team including named GPs, pharmacist, community nursing team, therapists, mental health professionals etc.

Collaborative working relationships:

Work within the primary care team, contributing to leadership of service evaluation to promote quality improvement activity.

Work in partnership with members of the PCN workforce including medical, nursing, pharmacy, PCN care co-ordinators, link worker, health coaches to personalise care

Management:

Use resources effectively to manage clinic delivery in line with local and local guidance and makes recommendations for change where improvements can be made.
