Main duties of the job

All aspects of nursing care from chronic disease management ,annual review ,health education and health promotion and wound care . All vaccination programmes for adults and children .

Adhereing to infection control policies .

Delivering care to vulnerable patients and helping to reduce health inequalities to improve care for all

We are a small 9000 town centre practice and pride ourselves on our friendly open atmosphere.We are an emis practice

We have 3 partner gps salaried gps , 2 clinical nurse practitioners 1 emergency care practitioner . We have 3 pharmacists 1 physicians associate, care coordinator , physio and social prescriber and young persons mental health workwer We are a member of a PCN with 2 other similar practices but we maintain our individuality . We offer a full range of GMS services. Training available for aspects of the role not familair to applicant

Job description

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

To be instrumental in the support of the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice manage

To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice

To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

To develop, implement and embed health promotion and wellbeing programmes

To implement and evaluate individual treatment plans for chronic disease patients that promote health and

To support the team in dealing with clinical emergencies and to maintain emergency care troley and resus equipment.

To recognise, assess and refer patients presenting with mental health needs

To implement vaccination programmes for adults and children. To deliver the cervical screening programme

To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

To support the clinical team with all safeguarding matters, in accordance with local and national policies

To deliver opportunistic health promotion where appropriate

  • Registered general nurse

Desirable

  • previous experience in district nurse or practice nursing an advantage but all training will be given
