Job summary
As a PCN pharmacist you will take responsibility for medicines optimisation within the PCN, medicines reconciliation, the provision of expert advice on clinical medicines and the undertaking of structured clinical reviews with patients to proactively manage their complex polypharmacy needs
Main duties of the job
The PCN are looking for a Clinical Pharmacist who will be an integral part of the General Practice team, working in Burn Brae Medical Practice, as well as part of a wider pharmacist network.
They will improve patients health outcomes and the efficiency of the Primary Care team by providing direct, accessible and timely medicines expertise.
They will work to optimise medication issues to improve patient care, safety and support clinical staff in the management of patients.
They will need to have the ability to influence and communicate effectively with clinical colleagues, patients and support staff.
The post holder will be supported to develop their role to become a non-medical prescriber.

A vacancy has arisen to join our mature Primary Care Network serving the populations of 12 practices across West Northumberland. The opportunity is to be part of an experienced team bringing the valued skills and knowledge of Clinical Pharmacists to the high achieving practices across the Tyne Valley.
The post-holder will join the network team taking a lead on the Medicines Optimisation agenda within practices and across the network, supporting staff with all aspects of medicine management and building links with community and secondary care pharmacy partners. As a patient-facing clinical role you will also have the opportunity to bring your skills to a wide range of clinical situations and support the practice team with long term condition management.
This post will be based at Burn Brae Medical Practice and at least one other GP Practice within the Network.
You will work within your professional and clinical boundaries whilst being supported with supervision from a named GP.
Candidates should be a registered pharmacist with 2-3 years post-registration experience, have good communication skills and value a patient-focused approach. Candidates should have completed or be working towards completing the CPPE pathway. A prescribing qualification is desirable but can also be supported.
The role is pivotal to improving quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. There will be opportunities to develop your role and skill-set in line with network aims and objectives.
