Job summary

The purpose of this role is to lead improvements to maximise best practice including safe, cost effective prescribing to improve the quality of patient care. The post holder will help patients to get the best from their medicines through timely medicines reconciliation after discharge and on other transfers of care, medication use reviews, identification of patients suitable for electronic repeat dispensing, promotion of online ordering, providing assurance of monitoring of high risk drugs, and handling of prescription queries within the areas of competence. In addition, the post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in line with therapeutic developments. This role is pivotal to improving quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice.

The successful candidate will be expected to enrol on to the 18 months CPPE PCPEP pathway ( if not already competed )and will be supported by us to complete this.

Main duties of the job

Support and work under the direction of our Clinical Pharmacist

Medicines reconciliation post discharge and on other transfers of care

Undertake medication use reviews

Day to day handling of prescription queries within area of own competence

Use and maintain information reporting systems and databases relevant to the position including incident reporting, medicines optimisation, prescribing data, cost savings, and patient outcomes

Participate in practice meetings and other meetings to improve engagement of the role of pharmacy technicians and to promote issues relevant to prescribing and medicines optimisation

Liaise with other pharmacy stakeholders e.g. Medicines Optimisation team, community pharmacy, and hospital pharmacy teams

Undertake, participate and share the outcomes of medication audits and quality improvement projects to facilitate improvement and changes in practice

Contribute to the review and development of repeat prescribing protocols and standard operating procedures to improve the efficiency and effectiveness of repeat prescribing

Ensure that all major MHRA drug alerts, where appropriate and within levels of competence, are acted upon within practices over an appropriate time frame and to advise on any medication changes that occur as a result

Provide clinical and professional support to GPs and members of the team

About us

We are a friendly, well organised, training practice. The Practice has 4 GP Partners, 3 Salaried GPs, Paramedic Practitioner, Clinical Pharmacist, Social Prescriber, Mental Health Practitioner, 4 Practice Nurses, 3 HCAs and 13 admin staff.

  • BTEC/NVQ level 3 or equivalent
  • Experience of working as a qualified pharmacy technician
  • Minimum 2 years post qualification experience
