Job summary
To
work on a local health initiative project to tackle inequalities of children and
families of Fleetwood. You will be part of a research project to
understand the wider determinants of health inequality & social
disadvantage within the family system impacting upon a childs 0-16yrs old A&E
admissions. The project will respond to
0-16yr old A&E admissions who fall within the core 20 plus population. The
projects aims are to;
Understand
the wider health inequalities and both the health and social needs of the whole
family system that maybe impacting upon the childs A&E admission.
Identify
and address the unmet health needs of the whole family system.
Provide/deliver/remove
barriers to health interventions e.g. Enhanced Health Checks, vacs, access to
drug and alcohol/Mental Health services (target list to be compiled with GPs)
Provide
up to 6 x family outreach visits to remove pre-existing barriers to access
health care
To support the practice nursing team in the delivery of nursing
services, working as part of the practice multidisciplinary team, delivering
care within their scope of practice to the entitled patient population.
The healthcare assistant will deliver and assist clinical staff in the
provision of treatment, preventative care, health promotion and patient
education.
Main duties of the job
Fleetwoods Health
inclusion plan will primarily target children and young people who attend
A&E in crisis as a gateway to reach the whole family system around the
child that sits within the core 20+ population of Fleetwood. Through this
gateway the project will allow us to reach the hardest to reach population, tackle
the inequalities and remove the barriers they face to accessing local health
care with its aim to improve the overall familys health and wellbeing an
improve health outcomes of the overall community.

Feetwood PCN are recruiting for an Health Care Assistant
to join their team. The successful candidate will join a team consisting of a pharmacist, pharmacy technicians, Paramedics, Social prescribers, Care coordinators and a robust Mental Health team supported by a team of GPs and clinical leads all striving to support the needs of the local community.
In this role you will have a mixture of work independently, making professional, autonomous decisions as well as working as part of a true MDT with a range of clinicians to improve health outcomes for patients.
The role will include face to face, telephone triage, assessing the patients needs with health care professionals, social workers, and PCN staff.

Fleetwood PCN covers 28,700 patients over three practices in the Fleetwood area and uses a network of skilled clinicians to forge relationships with varying healthcare providers and promote collaborative working through the 3 partner practices, providing personalised care for patients through every stage of life.
