Job summary

Docklands Primary Care network (DPCN) are looking for a new member of the team to work across our three member practices (E16 Health/Albert Road and Pontoon Dock, Royal Docks Medical Practice and Tollgate Medical Centre). Collectively we provide primary care services to approaching 50,000 patients in and around south of the Borough of Newham.

The care coordinator with data emphasis (CCD) will play an important role in pro- actively managing the GP and PCN Enhanced Service Contracts to ensure we are meeting all the contractual clinical requirements across the network and achieving the required targets on an annual basis.

Care coordinators play an important role within a PCN to proactively identify and work with our patients, including those with long-term conditions, the frail/elderly and other groups that may be subject to health inequalities.

Through local population health data searches, you will help identify, develop, and shape health interventions for implementation in our PCN and the wider health and care system, that will not only support and improve patient care but also achieve our PCN contract targets.

Care coordinators provide time, capacity and support to patients in preparing for or following-up clinical conversations they have with primary care staff (doctors and nurses) to enable them to be actively involved in managing their care. The aim is to help people improve their quality of life. This role includes advocating for patients who may need more support

Main duties of the job

This role is an integral part of the PCNs multidisciplinary team, working alongside the Clinical Director, PCN Manager, practice-based teams, Social Prescribers, and other Care Coordinators to provide all-encompassing support, promoting and embedding the personalised care approach across the PCN.

Supporting the GPs and PCN management team with vital administrative processes and clinical audits / reviews, working closely with GPs, care coordinators and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to patients and their carers.
