Job summary

This role will be inline with theGPN Foundation Programme. This is a 12 month training programme that will introduce the successful candidate into General Practice Nursing.

The Practice Nurse will be responsible for a number of clinical areas such as chronic disease management, health promotion, Immunisation, Cervical screening as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

Main duties of the job

a. Train to assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

b. Train in all Health promotion and wellbeing programmes

c. Train to implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

e. Train, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

f. Train to manage chronic conditions

i. Train to provide the provision of travel medicine services

j. Phlebotomy when required

k. Training in Chronic Disease & referrals

l. Maintain accurate clinical records

m. To ensure read codes are used effectively

q. To prioritise health issues and intervene appropriately

r. To support the team in dealing with clinical emergencies

t. Training for implementation of vaccination programmes for adults and children

u. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

v. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

w. To support the clinical team with all safeguarding matters, in accordance with local and national policies

Cwmbran Village Surgery situated in Cwmbran, Torfaen, Wales - nestled on the Monmouthshire-Brecon Canal, just North of the city of Newport. The practice is an easy drive from Cardiff (30mins) and North Bristol (35mins), there are also excellent rail links with regular services to Newport, Cardiff and to the North. The surgery benefits from ample parking and EV charging on site. We are a GP training practice and thrive in working with all training needs to encourage people reach their potential.

Job responsibilities

The following are the core tasks of the Trainee Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

a. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

b. Health promotion and wellbeing programmes

c. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

d. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

e. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

g. To prescribe medication for patients in accordance with PGDs and own scope of practice

h. To review patient medications to enhance compliance

i. To provide the provision of travel medicine services

j. To request pathology services and process pathology results as required

k. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

l. To maintain accurate clinical records in conjunction with extant legislation

m. To ensure read codes are used effectively by all members of the nursing team

n. To ensure the appropriate maintenance and use of chronic disease registers

o. To ensure compliance with the practice chaperone policy

p. To assist GPs with minor surgery when required

q. To prioritise health issues and intervene appropriately

r. To support the team in dealing with clinical emergencies

s. To recognise, assess and refer patients presenting with mental health needs
