This role will be inline with theGPN
Foundation Programme. This is a 12 month training programme that will introduce the successful candidate into General Practice Nursing.
The Practice Nurse will be responsible for a number of clinical
areas such as chronic disease management, health promotion, Immunisation,
Cervical screening as well as actively supporting the practice management team
in the reviewing and delivery of clinical policy and procedure.
Main duties of the job
a. Train to assess the needs of patients ensuring the provision of healthcare is
appropriate, incorporating evidence-based practice
b. Train in all Health
promotion and wellbeing programmes
c.
Train to implement and evaluate individual treatment plans for chronic disease patients
that promote health and wellbeing
e.
Train, to provide routine nursing care to patients in accordance with
clinical based evidence, NICE and the NSF
f. Train to manage
chronic conditions

Train to provide
the provision of travel medicine services
j.
Phlebotomy when required
k.
Training in Chronic Disease & referrals
l.
Maintain accurate clinical records
m. To ensure read codes are used
effectively

Cwmbran Village Surgery situated in Cwmbran, Torfaen, Wales - nestled on the Monmouthshire-Brecon Canal, just North of the city of Newport. The practice is an easy drive from Cardiff (30mins) and North Bristol (35mins), there are also excellent rail links with regular services to Newport, Cardiff and to the North. Nurse. There may be, on occasion, a requirement to carry out other tasks. This
will be dependent upon factors such as workload and staffing levels:
