Advanced Clinical Practitioner

Job summary

The Advanced Clinical Practitioner (ACP) will work autonomously in a primary care setting, using their advanced clinical skills to assess, diagnose, treat, and manage patients with a wide range of health conditions. The ACP will support the delivery of high-quality, patient-centered care and contribute to service development and the training of staff.

Main duties of the job

Key Responsibilities:

Clinical Practice

  • Undertake autonomous consultations, including assessment, diagnosis, treatment, and evaluation.

  • Manage both acute and chronic conditions, including minor illness and injury.

  • Prescribe medications within scope of practice and according to local and national guidelines.

  • Request and interpret diagnostic tests (e.g. blood tests, imaging).

  • Make appropriate referrals to secondary or specialist services.

  • Provide health promotion and disease prevention advice.

About us

Seven Dials Medical Centre proudly serves just over 7,000 patients in Brighton and Hove and has 14 members of staff including 3 GP Partners, an ACP, Practice Nurse and HCA. We work closely with the PCN to offer patients access to Social Prescribers, Mental Health Co-ordinators, Care Co-ordinators and a Pharmacy team.

We work hard to deliver the best levels of care to our patients and pride ourselves on a 'Family GP' feel. Many of our patients have been with the surgery for decades and we regularly receive positive feedback from patients both long standing and newly registered.

Our team is friendly and welcoming and the work environment is fun, professional and productive.

  • - Registered healthcare professional (e.g. RN, Paramedic, Physiotherapist)
  • - MSc in Advanced Clinical Practice or equivalent
  • - Independent Non-Medical Prescriber (V300)

Desirable

  • Excellent clinical reasoning and decision-making skills
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
  • IT proficiency and familiarity with clinical systems (e.g. SystemOne, Emis)

Experience

Essential

  • - Significant post-registration experience
  • - Proven autonomous practice in patient assessment and management
  • - Experience in leadership or supervisory roles an advantage

Desirable

  • - Experience in a primary care setting desirable
