Job summary
The Advanced Clinical Practitioner (ACP) will work autonomously in a primary care setting, using their advanced clinical skills to assess, diagnose, treat, and manage patients with a wide range of health conditions. The ACP will support the delivery of high-quality, patient-centered care and contribute to service development and the training of staff.
Main duties of the job

Clinical Practice
Undertake autonomous consultations, including assessment, diagnosis, treatment, and evaluation.
Manage both acute and chronic conditions, including minor illness and injury.
Prescribe medications within scope of practice and according to local and national guidelines.
Request and interpret diagnostic tests (e.g. blood tests, imaging).
Make appropriate referrals to secondary or specialist services.
Provide health promotion and disease prevention advice.
About us
Seven Dials Medical Centre proudly serves just over 7,000
patients in Brighton and Hove and has 14 members of staff including 3 GP
Partners, an ACP, Practice Nurse and HCA. We work closely with the PCN to offer
patients access to Social Prescribers, Mental Health Co-ordinators, Care
Co-ordinators and a Pharmacy team.
We work hard to deliver the best levels of care to our
patients and pride ourselves on a 'Family GP' feel. Many of our patients have
been with the surgery for decades and we regularly receive positive feedback
from patients both long standing and newly registered.
