Job responsibilities

The post holder is GP who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the PCN. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

The Candidate will:

Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.

Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.

Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.

Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example Advanced nurse practitioners and the personalised care roles.

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

Anticipate barriers to communication and take action to improve communication.

Maintain effective communication within the urgent care service and with GP practices and external stakeholders.

Act as an advocate for patients and colleagues.

Ensure awareness of sources of support and guidance (e.g. PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

The post holder will maintain the security of sensitive personal and other confidential information and will work within all relevant Information Governance policies reliably to working practice. Additionally, they will be expected to follow secure operating procedures for handling information in all formats, including verbal, written and that held electronically.

The post holder will have the appropriate level of child protection and adult safeguarding knowledge, skills and practice required for the post; be aware of and comply with child protection and adult safeguarding policies and procedures.

This post will be subject to a DBS (Disclosure and Barring Service) Check

In addition to observing the host Practices rules and procedures, which all staff are required to observe and follow, the post holder is also required to follow general policies and procedures that apply to the employment relationship. It is the duty of the post holder to accept personal responsibility for the practical application of these policies, procedures and standards.

Person Specification

Qualifications

Essential

• MRCGP / Relevant clinical qualification
• A medical practitioner whose name is included in the General Practitioner Register under article 10 of the General and Specialist Medical Practice Order 2003
• Not subject to suspension under section 41A of the Medical Act 1983
• Qualified General Practitioner (completed certificate of Completion of Training CCT)
• Currently on a performers list and not suspended from that list or from the medical register

Experience

Essential

• Experience In practice as a General Practitioner/Locum Practitioner
• Experience or additional training in clinical supervision
• Knowledge of quality improvement methodologies
• Excellent clinical assessment and decision-making skills
• Strong communication and interpersonal skills
• Ability to work under pressure and manage multiple priorities
