Job summary

Sangam Surgery, based in Newham, has an excellent opportunity for a committed GP to join our professional, innovative, and forward-thinking team. We are a progressing practice with a multi-skilled healthcare workforce and a shared mission to deliver the highest standard of patient care.

We are seeking a GP who can offer 4- 6 sessions per week and contribute to our supportive, patient-centred environment. The post-holder will manage their own caseload and address a broad range of health needs within primary care, ensuring safe, compassionate, and evidence-based care for both registered and temporary patients.

You will be supported by experienced clinicians including Doctors, Diabetic Specialist Nurse, Physiotherapist, Pharmacists, Nurses, Health care Assistant and an efficient administrative team, with opportunities for ongoing professional development and involvement in service improvement. This role is ideal for both newly qualified and experienced GPs who value teamwork, continuity of care, and high-quality clinical standards.

Main duties of the job

As a Salaried GP you will be responsible for:

a)Patient Care

Manage a personal caseload of patients, including registered and temporary patients.

Provide high-quality, evidence-based, patient-centred care for acute and chronic conditions.

Conduct consultations in person, by phone, and digitally as appropriate.

b)Clinical Duties

Complete care plans, medication reviews, and chronic disease management in line with best practice.

Prescribe medications safely, following NHS protocols.

c)Collaboration and Teamwork

Work closely with GPs, nurses, pharmacists, and other team members to ensure coordinated care.

Participate in peer support, case discussions, and clinical audits.

d)Administrative Responsibilities

Maintain accurate patient records using EMIS Web, Accurx, and CEG templates.

Complete documentation promptly to support governance and efficiency.

e)Practice and Professional Development

Attend weekly meetings covering governance, business, and clinical discussions.

Participate in teaching, training, and supervision of students where appropriate.

Engage in CPD and appraisals to maintain and enhance skills.

Job responsibilities

Clinical Responsibilities:-

- In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion.

- Making professional, autonomous decisions in relation to presenting problems, whether self- referred or referred from other health care workers within the organization.

- Assessing the health care needs of patients with undifferentiated and undiagnosed problems.

- Screening patients for disease risk factors and early signs of illness.

- In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.

- Providing counselling and health education.

- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.

- Recording clear and contemporaneous consultation notes to agreed standards.

- Collecting data for audit purposes and complying with QOF requirements.

- Delivering enhanced services.

- Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).

- Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate

- In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

Other Responsibilities within the Organisation:-

- Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety.

- A commitment to life-long learning and audit to ensure evidence-based best practice.

- Contributing to evaluation/audit and clinical standard setting within the organisation.

- Contributing to the development of computer-based patient records.

- Contributing to the summarising of patient records and Read-Coding patient data.

- Attending training and events organised by the Practice or other agencies, where appropriate.
