Job summary
Saffron Health Partnership is looking for a Practice Nurse who is trained in Diabetes management to join our committed and hard-working team.
The Practice Nurse is an
important role within the practice's Nursing Department. The Nursing Department
is led by the Advanced Clinical Practitioner and consists of a Nurse
Practitioner, Emergency Care Practitioners (Paramedics), Practice Nurses, HCAs
and a Phlebotomist.
Main duties of the job
To provide practice nursing services and support to patients including treatment, screening, chronic disease management (in particular Diabetes) and advice within scope of competence.
Duties must be carried out in accordance with the NMC Code of Professional Conduct and with regard to practice protocols and procedures.
About us
We are a long-established practice with approximately 22,000 patients. We are a friendly and supportive team comprising of 55 staff members serving patients across Biggleswade, Sandy, Langford and the surrounding areas. This role will require working across all 3 sites.
The practice is also a member of Sandhills Primary Care Network with which it has a positive and collaborative working relationship.

Job responsibilities
- Provide
a high standard of patient-centred general practice nursing care to the
practice's patient population
- Initiate
and maintain health screening and promotion services for relevant cohorts in
the practice's patient population, recognising where modification of processes
or referrals are necessary
- Carry
out treatment room duties for relevant patients
- Manage patients
with chronic disease, in particular Diabetes, making use of patient recall
systems
- Liaise
with healthcare colleagues, both internal and external, and other agencies to
assure continuity of care when necessary
- Maintain high
standards of infection control including supporting colleagues with implementing
infection control measures when needed
- Be competent in
anaphylaxis and resuscitation techniques and adhere to the requirement to attend
refresher training annually
- Maintain an
up-to-date working knowledge of national and local screening and vaccination programmes
- Maintain accurate, contemporaneous patient records using Systm One clinical software
- Prepare specimens for laboratory collection
- Assist in the teaching and mentoring of student
nurses
- Assist
in clinical audit,
and the design and implementation
of systems to monitor and evaluate standards of care
- Take an active role in development and
updating of protocols and procedures relevant to role
- Contribute to effective team working both
within the Nursing Department and across other departments in the practice
- Participate
in training and/or induction of colleagues as appropriate
- Be available to
support non-clinical colleagues (e.g. receptionists) with clinical queries
within scope of practise
- Maintain continuous professional
development in keeping with role
