Job summary

Saffron Health Partnership is looking for a Care Navigator to join our committed, and hard-working team.

The role of the Care Navigator is part of the Patient Services Team. The Team is led by the Patient Services Manager with the support of a Team Leader and a Senior Care Navigator. We are a very busy practice, and the role includes working on reception as well as within the office.

35 hours per week: Monday 08:00-18:30, Tuesday 08:00-18:30, Wednesday 13:30-18:30 and Friday 08:00-18:30

28 days leave plus Bank holidays

NHS Pension Scheme (14.38% employers contribution)

Main duties of the job

- Answer, deal with and navigate incoming requests from patients, using a complex appointment system

- Deal with in-person patient enquiries at a busy reception desk

- Provide basic and accurate information in-person and via phone/email

- Maintain office security by following safety procedures and controlling access via the reception desk

- Perform other administrative duties such as emailing, photocopying, and scanning documents

- Accepting, issuing, and handing out letters, completed forms, specimen bottles

- Advising patients of practice procedures

- Registering new patients

We are a long-established practice with approximately 22,000 patients. We are a very friendly team serving patients across Biggleswade, Sandy, Langford and the surrounding area. This role is based across all 3 sites with staff parking facilities available.
