Partnership is looking for a Care Navigator to join our committed, and
hard-working team.
The role of the Care Navigator is part of the Patient Services Team. The
Team is led by the Patient Services Manager with the support of a Team Leader
and a Senior Care Navigator. We are a very busy practice, and the role includes
working on reception as well as within the office.

hours per week: Monday 08:00-18:30, Tuesday 08:00-18:30, Wednesday 13:30-18:30
and Friday 08:00-18:30

Main duties of the job
- Answer, deal with and navigate incoming requests
from patients, using a complex appointment system
- Deal with in-person patient enquiries at a
busy reception desk
- Provide basic and accurate information
in-person and via phone/email
- Maintain office security by following safety
procedures and controlling access via the reception desk
- Perform other administrative duties such as
emailing, photocopying, and scanning documents
- Accepting, issuing, and handing out letters,
completed forms, specimen bottles
- Advising patients of practice procedures
- Registering new patients

We are a long-established practice with approximately 22,000 patients. We are a very friendly team serving patients across Biggleswade, Sandy, Langford and the surrounding area. This role is based across all 3 sites with staff parking facilities available.
