Job summary

Responsible for the assessing, diagnosing, prescribing and treating of patients who are registered within the GP practice as well as for non registered patients from other practices whom the practice has responsibility for through any of its enhanced services schemes.

Main duties of the job

Working as an autonomous nurse practitioner as part of the GP Practice in the delivery of medical care. The post holder will be a clinical expert working across traditional boundaries providing a comprehensive assessment of patients with undifferentiated or undiagnosed conditions. The nurse will see and treat the patient and provide valuable healthy lifestyle information to promote good continued health.

1. To see patients independently. To diagnose and treat within the setting of the GP facility.

2. Assist in the development of an integrated approach to the delivery of patient care involving all relevant stakeholders including health and social care organisations, particularly within the primary care field

3. To provide telephone assessment to patients calling Coseley Medical Centre and inviting them to attend the surgery if appropriate.

4. To provide clinical expertise to patients walking in to, or with appointment at Coseley Medical Centre, by assessing and treating their medical condition.

5. To provide clinical support to colleagues at CMC.

6. Continue to develop, expand, cascade and role model own personal clinical expertise as autonomous/independent practitioner.

Job responsibilities

Main duties and responsibilities:

Working alongside the nurses and doctors at the surgery the post holder will be responsible for the following:

1) Clinical

1. To see patients independently. To diagnose and treat within the setting of the GP facility.

2. Assist in the development of an integrated approach to the delivery of patient care involving all relevant stakeholders including health and social care organisations, particularly within the primary care field

3. To provide telephone assessment to patients calling the surgery and inviting them to attend the surgery if appropriate.

4. To provide clinical expertise to patients walking in to, or with appointment at the surgery, by assessing and treating their medical condition.

5. To provide clinical support to colleagues at the surgery.

6. Continue to develop, expand, cascade and role model own personal clinical expertise as autonomous/independent practitioner.

7. In partnership with others, challenge and critically evaluate the boundaries of autonomous practice, such that patient access, choice and outcomes will improve.

8. Help in the development and evaluation of clinical guidelines. Involved in team discussions to ensure best practice

9. Develop own areas of specific clinical expertise in order to link acute, primary, community and emergency care sectors in expanding nursing practice within the surgery.

2) Organisational

1. To take clinical responsibility for themselves and to provide assistance and supervision to other health professionals as required.

2. To assist in the maintenance of clinical standards and to share in the efficient running of the service.

3. To share the responsibility for the security and care of equipment.

4. To be aware of the Political and Social issues which affect the delivery of healthcare.

5. To help in the development of policy and procedure within the surgery.

3) Communication

1. To maintain accurate patient records for users of the service, using the designated software system for the practice.

2. To be aware of responsibilities under the Data Protection Act and Caldicott principles.

3. To attend regular team meetings.

4. To ensure effective communication with colleagues and the wider health care community as appropriate when referring a patient.

5. Working with interpreters [link workers] for non-English speaking patients.

6. Record and report all clinical incidents to Nurse Lead.

7. Assess individual communication needs and understanding for informed consent.

4) Education, Training & Research

1. Contribute to data gathering as needed for QOF, enhanced services and KPI targets.

2. To take responsibility for own professional development undertaking training for specific clinical tasks

3. Participate in clinical audit. For Clinical Governance and Training purposes a random sample of the clinical notes written by every clinician (nurses and doctors) will be audited regularly against a set of criteria. These criteria include clarity and accuracy of documentation as well as standards of good clinical practice.

4. Participate in research programmes as appropriate.

5. Participate in the development and implementation of standards and audit activities.

5) General

The responsibilities of the post will be the subject of on-going review between the post holder and line manager. The post holder may be required to undertake other duties to meet the needs of this developing service.

1. The Post Holder will be required to work flexibly, which may include additional hours, to cover holidays and sickness.

2. To maintain active NMC registration

3. To adhere to Organisational policies

4. To ensure confidentiality at all times, only releasing confidential information obtained during the course of employment to those acting in an official capacity in accordance with policies and Caldicott guidelines.

5. Attend all mandatory training courses and the updates and any study days as appropriate to the post

6. Attend performance and development reviews (PDR) with your line manager. You will have a PDR at least once a year and twice during the probationary period with the line manager. This process will include the development of a Personal Development Plan.

7. To carry out other duties commensurate with the grade as directed.

8. Attend meetings as requested by the practice leads.

9. Establish and maintain effective working relationships with co-workers, supervisors and the general public.

10. Maintaining regular consistent and professional attendance, punctuality, personal appearance and adherence to relevant health and safety / environmental procedures.

11. To have a good understanding of company policies and procedures as laid out in the surgery and Conditions and Resource Pack
