Job summary
Tamworth House Medical Centre is looking for an Advanced Nurse Practitioner. We are a friendly, welcoming, and
innovative GP Practice in South West London delivering care to over 9,600
patients, achieving high QOF performance and maintaining a Good CQC-rated
service.
We are seeking an enthusiastic, motivated and forward-thinking Advanced
Nurse Practitioner who enjoys general practice. We offer NHS Pension, Paid indemnity cover and standard annual leave and study leave.
We welcome applicants wanting part time or full time employment.
Post will be available from 01 March 2023.
Main duties of the job
The successful candidate will act as an autonomous
practitioner working independently and in conjunction with other health care
professionals to assess, diagnose and treat the conditions of patients
attending within primary care initiating direct referrals as appropriate.
The candidate will have an Independent prescriber qualification. GP practice
experience is desirable.

Merton providing NHS primary care services to over 9600 patients. We have a
well-established team of 4 GP Partners, 2 salaried GPs, an Advanced Nurse
Practitioner, 1 Practice Nurse, and an HCA.
We are a training practice with medical students, nursing
students, GP registrars, and Foundation Year 2 Doctors.Tamworth House Medical Centre is an active member of the East Merton Primary Care Network (EMPCN) and
is involved in new projects to change the delivery of Primary Care. We have
recruited prescribing pharmacists, paramedics, physician associates, and Health
and Wellbeing Coach to expand the care we provide to our patients.

- Work as part of a multi-disciplinary team to deliver safe and high
quality care.
- Carries out clinical nursing practice at an advanced level using
expert clinical skills, critical judgements & knowledge to deliver
appropriate and safe care.
Delivery of
care in line with NSF, NICE guidelines and evidence-based care
- Triage and treat patients, performing specialist assessment of
their needs and make referrals as necessary to other members of the
primary and secondary health care teams as appropriate.
Co-ordinate the
planning and delivery of care ensuring patients and carers/relatives are
involved.
- Prescribing and reviewing medication that is appropriate to patient
needs and in accordance with latest evidence-based practice, national and
practice protocols and within the scope of qualified practice.
- Diagnose and manage acute and chronic conditions, including drug
and non-drug based treatment methods into a management plan.
Request
diagnostic investigations and interpret within agreed protocols e.g. x-ray,
blood tests, scans, etc.
- Assess, diagnose and treat patients in practice premises or their
own home who require medical attention and/or refer to appropriate agency.
- Refers patients to other members of the multi-disciplinary
practice teams, secondary care organisations and other organisations as
necessary.
