About the Role

We are seeking a dynamic and motivated General Practitioner (GP) to join our expanding team under the Additional Roles Reimbursement Scheme (ARRS).You will work collaboratively within our team to deliver high quality clinical care to the patients registered and attending the GP surgery and work closely with the clinical leads and local practice team to support the delivery of the surgeries operational plan.

  • You will strive to maintain quality within the practice ensuring participation in clinical governance activity and contribute to the improvement in quality health outcomes through audit and the Quality and Outcomes Framework and contributing to the practice achieving other quality standards to include CQC standards. To support the development, delivery and review of the surgery action plan including list growth, patient satisfaction levels, QOF, enhanced services.
  • Provide clinical care to patients in line with a total triage model. Including consultation sessions (both in person and by video), telephone triage, correspondence, results, medication reviews and prescription signing, and other clinical administration, including safeguarding and palliative care cases, medicals, reports, home visits, surgery meetings. Home visits are rare, as we work closely with a dedicated home visiting team who support this aspect of care.
  • Adhere to the highest personal and professional standards as set out by the GMC and to put individual patient care as your priority.

About the PCN / Practice

Lambgates Health Centre is a well-established training practice located in Hadfield, Glossopwithin Derbyshire. We currently care for a growing list of 7,412 patients and are proud to serve our community from a setting surrounded by stunning scenery and convenient local amenities. As a dedicated training practice, we support the development of GP registrars, foundation doctors, and medical students, fostering a strong culture of learning and mentorship.

We maintain a close, collaborative relationship with Glossop PCN, which includes three other local practices. Our core values are collaboration, continuous learning, innovation, and delivering compassionate care.

As demand for our services grows, so too must our team. We are looking for someone who shares our values and is eager to contribute to our supportive, forward-thinking practice environment.

We are in the early stages of a practice merger with a neighbouring practice within the Glossop PCN- cross site working may be required.

Job description

Job responsibilities

The post-holder will provide comprehensive primary care services, including surgery and telephone consultations, home visits, repeat prescriptions, and related administrative duties. They will exercise professional autonomy in assessing, diagnosing, and managing patients with undifferentiated health problems, while promoting preventative care through screening, counselling, and health education. Responsibilities include developing patient care plans, maintaining accurate clinical records, meeting QOF and audit requirements, and delivering enhanced services. The role also involves prescribing in line with practice formularies and adhering to all standard duties expected within general practice.

Other Responsibilities within the Organisation:

The post-holder will comply with all relevant practice policies, including prescribing, confidentiality, data protection, and health and safety. They will engage in learning, participate in audits, contribute to evaluation and clinical standard setting, support the development of computer-based patient records, and ensure accurate summarising and coding of patient data.

Attendance at training and professional events organised by the Practice or external agencies is expected.

Communication: The post-holder will communicate effectively with patients, carers, and team members, recognising and responding to the need for alternative methods of communication as appropriate.

Contribution to the Implementation of Services: The post-holder will apply Practice policies, standards, and guidance, discuss with the team how these affect their work, and participate in audits where appropriate.

Confidentiality: The post-holder will respect and maintain the confidentiality of patient, staff, and organisational information, sharing data only with authorised personnel according to practice policies and legal requirements.

Health and Safety: The post-holder will promote and maintain their own and others health, safety, and security, including identifying and managing risks, following infection control procedures, maintaining safe work areas, and reporting potential hazards.

Equality and Diversity: The post-holder will support equality, diversity, and the rights of patients, carers, and colleagues by treating everyone respectfully, non-judgmentally, and in accordance with relevant legislation and practice policies.
