Job summary

We have an exciting opportunity for an enthusiastic Paramedic Practitioner to join our dynamic team following the retirement of our paramedic. This permanent position is well suited to individuals who are enthusiastic about providing high quality support to primary care.

Lambgates Health Centre are looking to recruit a further full time 30 hours per week Paramedic to provide a high-quality service to our patients.

Working days Tuesday, Wednesday, Thursday & Friday

Working:-

2 early shifts 9.30 5:00 (1/2 unpaid lunch)

2 late shifts 10:00 18:30 (1/2 unpaid lunch)

Main duties of the job

  • The role will be to provide a specialist Paramedic resource for health care professionals and service users, working in collaboration with other members of the Multidisciplinary Team.

  • To work as part of a clinical team, with GPs, ANPs, MHP Practice Nurses, and Healthcare Assistants within the practice.

  • To be responsible for seeing patients within the practice and visiting patients in their home, who present with a wide range of minor illness, urgent and same day requests for treatment and chronic disease management, providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

  • The post holder will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. There will be the need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations

  • To work within the community, as an autonomous, accountable, Paramedic Practitioner, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment, to deliver quality patient services.

  • To assess, diagnose, treat, refer, or signpost patients who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.

We are a well-established training practice for GP registrars, foundation doctors and medical students with a growling list size of 6950 patients situated in the beautiful town of Hadfield in the High Peak of Derbyshire close to local amenities, surrounded by spectacular scenery. We have a very cohesive working relationship with Glossop Primary Care network which includes a further 4 practices. Work as part of a multi-disciplinary team (MDT) within Lambgates Health Centre

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national)and local clinical guidelines and local care pathways) or make necessary.

Perform investigatory procedures as required and undertake the collection of pathological specimens including intravenous blood samples, swabs and other samples within their scope of practice and within line of local and national guidance.

Support the delivery of anticipatory care plans and lead certain services (e.g., monitoring blood pressure and diabetes risk of elderly patients)

Provide an alternative model to urgent and same day GP home visit for the practice.

Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered.

Communicate proactively and effectively with all colleagues across the MDT, attending and contributing to meetings as required

Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient healthcare and registration with the organisation

Communicate effectively with patients and, where appropriate, family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices

Assist with the annual flu immuinisation programme.

Patients with problems needing referral to secondary care should be discussed with the GP supervisor (or GP on call in their absence) before making such referral.

Works from the surgery and within communities as an autonomous practitioner caring for patients and families.

Works as an autonomous practitioner, in accordance with the Health Professions Council. Ensure that personal and professional clinical standards are maintained.

To undertake assessment for patients within the community and those attending the surgery, using diagnostic skills and initiation of investigations where appropriate.

Prescribe/issue medications as appropriate following policy, patient group directives and local pathways.

To formally and informally impart knowledge and skills to colleagues promoting peer review and best practice within the work environment.

To maintain accurate, contemporaneous healthcare records appropriate to the consultation.

Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high quality patient care.

Works with local policies and procedures.

Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service.

To achieve and demonstrate agreed standards of personal and professional development in order to meet the needs of the service.

  • Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator.
